46 Year Old with Volume Loss in Temples, Lower Face and Chin - Spokane, WA
ORIGINAL POST
I had gone to another provider for voluma (cheeks)...

I had gone to another provider for voluma (cheeks) and juvaderm (lips, marionette lines), at first I was quite please but over time I developed a large ridge under both of my eyes. The other provider recommended that I have my tear troughs filled but she wasn't comfortable doing it. I researched plastic surgeons in my area and found great reviews for Dr. Chestnut...I scheduled a consult for my tear trough area. At my consult Dr. Chestnut was very upfront and honest, letting me know that the original filler was placed too high and that the "ridges" were actually filler and that some of it needed to be dissolved before we could proceed, I was shocked but I appreciated his cantor. He examined my face and pointed out that I needed volume in other areas (temples, lower cheek) to give me the youthful look I wanted...He definitely put me at ease with his kind bedside manner and understand of my face. I decided to have the filler dissolved (which he did for free) and go with the Sculptra. To my surprise, he called me later that day to see how I was feeling which is a testament to his overall professionalism and concern for his patients. I went in yesterday (4.6.15) and had the Sculptra done along with Perlane in my chin. I brought a list of concerns and questions for Dr. Chestnut and he sat with me until each and every one of them were answered, I brought photos for him to look at.. I thought it was silly because they were 12 years old but he was very appreciative. I mentioned other procedures that I was considering and he said that I didn't need them which I thought was amazing because it just proved to me that his concern was me and my happiness not "upselling" products or procedures that I didn't need. I am so happy with my outcome, my chin looks amazing and the fullness in my face is just perfect (no pillow face), I realize that it will take Sculptra time to work but if the results today are any indication of the results I will have in a few months I am one happy girl and I attribute that to Dr. Chestnut and his amazing nurses.

Looks awesome!! I just had voluma in my cheeks and on one side in my tear trough (to maximize symmetry) and fear I may be developing the shelf you refer to. It makes it look as though I have hollows under my eyes where I didn't before. What was the location of your injection, if you don't mind me asking? My injection site was under my cheek and he drove the needle up into the tear trough. I almost passed out. I was wondering if that was normal

Just want to be the best 46 yr old version of myself!!!

I can't say enough wonderful things about Dr. Chestnut and his staff at Dermatology Specialist of Spokane. Let me start by mentioning again I don't want to look 30, I want to be the best version of my 46 year old self. I have been working with Dr. Chestnut and his amazing nurses for quite awhile now and could not be happier. I've had 3 treatments of Sculptra and as you can see by my pictures my face looks fuller and quite natural. People that have not seen me in awhile tell me I look rested, and healthy but don't ask "what I've had done" which is exactly the results I was looking for. I attribute my fantastic results to the skill of Dr. Chestnut...He is knowledgeable, and truly a master of his craft. He has come in early and even met with me over lunch when I had some concerns...His bedside manner is outstanding, after each procedure he has called me after business hours to check on me. If you are looking for a Plastic Surgeon in Spokane, do yourself a favor and go to the BEST...Dr. Chestnut!!

You look absolutely amazing, ten years younger. Not that you looked bad before, but the difference is so subtle and natural, it is just you, looking very healthy and beautiful. I'm sure you must be thrilled!
November 27, 2015
You look fantastic, really natural. How far apart did you have the three treatments and when did you start to see the results as it is my understanding that Sculptra isn't instant? I have lost volume after a laser/IPL treatment and have been considering Sculptra.

THANK YOU very much, you're very kind - I appreciate you not mentioning my BRF (B*tching Resting Face)..I never planned on posting any pictures. I am absolutely trilled with the results!

Thank you very much Whatamistake - I appreciate your kind comment : ) I got my first treatment (2 viles-temples, mandibular, undercheekbone/cheek ) 7 months ago then another treatment (1 vile-temple, jawline) 3 months ago. The results are gradual but I'd say I started to see a difference after 3-4 months. As you can see by my before picture my right side was much more deflated then my left in the temple/cheek area, so the 2nd treatment was concentrated on those area. No one is completely symmetrical so I appreciate that my Dr. has a very skilled eye and knew exactly where it needed to be placed to give me the results that I wanted. I would definitely recommend Sculptra over a regular "filler" for the areas I needed it for. It is more costly upfront BUT actually more economical because it last much longer, and you won't need as much over time. The only thing I would recommend is that you make sure that your Dr. is very skilled in mixing/injecting Sculptra, as it isn't like other fillers. If you choose to do it, I hope you are as happy with the results as I am!!

How was recovery from sculptra? I am considering sculptra but am afraid of bruising and nodules. Did you have any issues with bruising or nodules?
November 28, 2015
The recovery was easy. I got one tiny bruise under my cheekbone that was easily covered with concealer and BB cream. I did not have any nodules but I believe that is because my Dr. is very skilled and I followed the aftercare instructions to the letter. When we first started discussing Sculptra I had never heard of it so I did a lot of research and brought my concerns to him. He said that the way the Sculptra is mixed and placed (basically on the bone) by the Dr. is very important but I also must massage the areas for 5 minutes 5x per day for 5 days. I will say that the injections are a bit uncomfortable because they go so deep but they are not "painful". If you have done your research on the Dr. and are comfortable they are experts in injecting Sculptra I'd go for it!! Good luck : )

Thank you!!!! So glad you had such good experience. You look amazing. One more question: do you know if the sculptra was mixed several days prior to your injections or on the day of. I keep reading it should be mixed 24-48 hours prior to injection to prevent nodules but my ps says he always mixes it immediately prior to injection and had good results. Just wanted to get any info you may have so I can make best choice on doctor. Thank you so much for your replies!!

During both my procedures it was mixed in the room. After he diluted it, his nurses continually flipped the syringes so the product was being mixed...It was probably 20 minutes before he came back to start the injections, the full syringes never stopped moving until they were used. The first treatment (2 viles) was diluted into 6 syringes and the 2nd treatment (1 vile) was in 3 syringes. In MY opinion, I think it really depends on the dilution of the product and not when it's mixed altho I have seen different posts regarding mixing it prior to the appt. When my PS first started talking to me about Sculptra, he was very adamant that finding a Dr. that is specifically trained was by far the most important thing. He told me that ANYONE (medi-spas, dentist etc.) can get training for fillers and Botox in an 8 hour seminar but that doesn't mean they know what they are doing or they understand the anatomy of the muscles in the face. I was going to a dermatologist for my botox, and she was fine, I told her I was considering do Sculptra and she said they did not do it at their facility because of the specialized skill needed (I appreciated her honesty). Maybe you can search their website and see if they have a PS in your area that they recommend. Please let me know what you decide to do!

Thank you for your detailed reply. The doctor I am planning to see comes highly recommended but I am concerned due to the laser damage - I don't know how Sculptra will react to the underlying damage. Fingers crossed I will be as happy as you are with your results!!
November 29, 2015
Why kind of damage are you talking about..How long ago was the laser treatment? I'm sure after you discuss your concerns with your PS he will be able to give you advice on how to proceed. Please let me know what he says and what you decided to do...Good luck!!

3 year update on Sculpra
HayesgangFebruary 2, 2018
It has been almost 3 years since I received Sculptra (originally injected 4..6.15) and I have to say it was the BEST investment I have ever made! At first the cost scared me a bit but it has legitimately lasted the 2+ years they said it would. I noticed in a recent photo that my left temple is a bit "deflated" and the other spots aren't as "full" as they once were. I will be calling to make an appt. with Dr. Chestnut within the next couple of months, I know that since Sculptra has gained popularity more "professionals" are doing it (saw an ad for it at my local spa) but I would NEVER consider having any one but Dr. Chestnut do my injections!! He has the best qualifications, an eye for symmetry and you can't beat his honest, upfront approach to fillers! If you want the BEST, look no further than Dr. Chestnut!!

Thank so much for your post. I had a similar problem with my own fat. The fat is now flattening and I am very happy about that.

You look great. What places were you injected?
February 14, 2018
did you post a 2018 photo? if you did, I’m missing it. if not, please do. the way the Sculptra diminished over time Would be instructive. thanks for your thorough narrative. .
March 28, 2018
Hi, sorry for the late reply! I didn't post updated pictures...My appt. for Sculptra is tomorrow (3.29.18) and I'll be posting new pictures. I am focusing on my temples because that is the only area that has "deflated" over the last couple years.
February 17, 2018
He did a great job. Your results are natural and very smooth. I had wondered about which long term filler would work the best and it looks like sculptra did the trick. You look very pretty!
March 28, 2018
Thank you very much Laura! Sculptra was the best investment I've made (besides Dysport of course :) I have an appt to have 1 vial done tomorrow (3.29.18) and I'll have my temples done again...They are the only area that has "deflated" over the last few yeas. I will be posting pictures.
I'm so glad to hear you are happy with the sculptra and perlane. We'd love to see some before and after pics to see what you are talking about!
Thats quite alright- we are just glad you came back and let us see :)
