Vertical Chin Reduction for a Long Square Chin - Sacramento, CA

I have a long face which doesn't bother me so much...

I have a long face which doesn't bother me so much. What has always bothered me is that my chin is long and square and I have never liked the masculine look of it. I finally went to a PS/maxillofacial surgeon for a consult. He didn't want to operate because he said I was "already attractive" and didn't need it but he also acknowledged that yes my jaw is excessively long. We finally decided together to do chin burring intraorally, no wedge reduction. He did not think doing a wedge reduction was a good idea because of how people look in the future as they age. He said a lot of doctors don't want to tell you that it can create a "step" on either side of your chin and you can prematurely age and have jowls. He said that he doesn't just think of right now but he thinks of his patient's future as well. I was very happy with his honesty and it made me feel comfortable proceeding. The trade off is that I know I will not have a huge change, it will be more subtle. This is because he said if he takes alot off I can end up with soft tissue sag since he will not be doing a submental incision where he would able to tighten it. I had my surgery yesterday. It took less than an hour and my head is wrapped until my appointment later this afternoon. After that he will give me a removable chin strap to wear 24 hours a day for the first week. Then I will wear it only at night for an additional week. I have a high tolerance for pain but it was pretty painful yesterday... sore and some sharp pains around my bottom teeth. I've been taking Norco and today I feel so much better. I can't wait to see the results. I can only hope that there is a noticeable difference and no soft tissue sag. I will post pics later.

Day 4 and wow am I swollen

AboutTime14
It is Friday, a full 4 days post surgery. My lower lip is swollen about 5x the normal size, no joke. It is HUUUGE. It is so big that the skin peeled off like a snake shedding skin. I literally woke up in the morning yesterday and all the skin was hanging off, leaving it very shiny (top lip is totally fine, not swollen at all). It makes it difficult to talk and to eat and I am definitely NOT presentable. Other than that it hasn't been bad. My jaw area is swollen like a chipmunk but nothing too bad. Its not that sore either. My bottom teeth are only slightly sore and the stitches don't bother me. It is just THIS LIP. I was not prepared for this part, although my doctor did say my lip could swell, but the extent is shocking to me. I would say it's 99% of why the healing process has been difficult for me, but everyone's experience is different. To be honest I haven't heard anyone else online complain about their lip, except minor swelling. So sounds like I may be one of the rare ones to have it be this bad? I haven't left the house since I got home and am bored out of my mind. I can tell it's gonna be another week or so before I'm presentable and my doctor agrees (he said about day 10). I only took 1 week off work. .. Definitely not enough. I will most likely be out next week too. So for others out there, take off at least 2 weeks. Make sure you have lots of food options.. mashed potatoes, raviolis, lasagna, ice cream, soups, apple sauce, pudding. . That's pretty much what I've been having.

Day 5, Still so swollen

AboutTime14
Saturday, 5 1/2 days after my surgery. Lip is exactly the same. I'm at the point of feeling like it will NEVER go down and I will always look this way. I know that sounds crazy and of course it will go away but it just doesn't feel that way! It is so huge I can barely eat or talk. I can literally see my lower lip when I'm just staring straight ahead. It's sore and feels like it's burning. Also, as the feeling to my chin is coming back, the pain is actually getting worse. My lower teeth hurt so bad last night and this morning. My chin tingles too and sometimes feels itchy but I don't know where it's itching from which drives me crazy. Beware that the middle of the night and morning are the WORST times for pain due to sleeping laying down and the swelling staying there. Try to sleep sitting up if you can.. it makes it sooooo much better. I've given up on the Norcos because it makes my breathing shallow and only works for the pain for about 2 hours. I found that strangely enough Excedrin has worked the best for me. It takes away the achiness but without feeling weird or nauseous. I'm doing warm salt water rinses after I eat too to keep the stitches area in my mouth clean.

Thanks for the information. I'm interested in getting a reduction myself. Can you post before and after pictures?
October 30, 2014
I will once the swelling goes down.. I want my pics to truly reflect the outcome. Right now I think my 'after' pics could scare someone off lol. But that doesn't mean I won't like it once I'm healed.

Okay, Thanks for respond!

How long do you think you'll have to wait before you post photos? I also feel like my face is too long, I think if my forehead was rounder my overall face wouldn't look as long. Btw I really enjoyed reading your posts, made me giggle a bit xD

Hi, do you have any after pics. I think you already have a beautiful jawline so just a slight reduction would look great on you.
October 30, 2014
I will definitely post 'after' pics as soon as the swelling goes down. I am still VERY swollen on the chin area and it's been 16 days since my surgery. So far it looks less square and shorter but it's also puffed up so much I can't tell what the end result will be.

Me too i just had a chin reduction last Thursaday. And my face is very swollen...like a huge rectangle. My boyfriend has been calling me Robert Zdar.....which is mean. Anyway my surgeon removed 5mm of bone i think and he did it under my chin so i will have a scar. Not worried about it too much though... Hopefully it will heal well. So anxious to see my final outcome. I keep touching the swollen area dont know if that will affect the result.

Mine also said he removed 5mm. To me, although it does look softer (not so square), it looks even bigger than before. I just have to keep reminding myself though that this is not possible and that I'm just swollen. Otherwise I start really panicking! The only thing I do notice though is that my labiomental fold (the part under your lip that dips in) looks sooo much deeper. I am hoping this is because my chin pad is swollen and not because he did nothing to my tissues and so they are being pushed up due to the bone reduction. It's so hard to remember what you looked like/felt like before surgery because now I'm used to seeing myself as I am. So I hope when my chin pad's swelling goes down the fold will be more shallow. My front view looks good, it's the side view now that looks kinda funny due to this. I'm hoping I didn't exchange one problem for another.

It will look bigger before you can see the results. I was made aware of this and I STILL keep freaking out. I'm 9 days post and the swelling is improving small amount day by day. I haven't had any loss of sensation in the lip and i was eating solids second day after the op which worried me because i read people say it takes 2 or more weeks for that. I think by week 3 I will be able to see how much improvement there is.
November 5, 2014
I'm now 22 days post op and still swollen. I was feeling feverish the other day so I went back to my doctor and he put me on other antibiotics just in case of an infection. my swelling has not went down in the last 10 days or so. It's just staying the same! But I know it's swelling because I can barely move my chin when eating or talking.. it just feels stuck and stiff. He also took out my stitches yesterday because they did not come out on their own but he said the incision looks great and that part really doesn't hurt. Just my chin itself is very tender. I can barely feel the middle of it and my lip still tingles. Seems like I'm having a harder recovery than most.
November 5, 2014
I think its because the tissues in the chin are very dense so swelling takes forever. Is your chin already looking shorter to you? Or does it still look the same just with extra swelling? My doc says 6 weeks till you see a difference.
November 5, 2014
It looks a tiny bit shorter but more than that it looks less square, so it's more feminine looking which is what I wanted. I would have loved for it to be way shorter but honestly with this type of surgery you can't expect much. It's so difficult for them to get a good result without actually breaking your jaw and setting it back. I wasn't willing to go through all that. So it's subtle, but but right now I can't even guess as to how I'll really look because of the swelling. From the side I look huge like a monkey jaw.. Not cute. I'm embarrassed to go out. I feel like everyone is staring, plus the fact it's so tight I talk funny and my smile (which was one of my best features) is horrendous.
November 5, 2014
Me to haha my smile is wonky i think the muscles are not relaxed.
June 13, 2015
Hi lady! How are you doing now? Can you please post an update? I'm thinking of having the same procedure done. Do you recommend it?
June 25, 2015
Hi! I am doing great now, almost 9 months post-op. The cons: I still have tightness across my chin and when I touch it there's still a bit of numbness. My doctor told me this should all be gone at 1 year, but I don't know if that's gonna happen cause it's pretty much stayed the same over the past 5 months or so. My lower lip also feels tighter than it did and I feel like the surgery kinda lifted my lip up. Hard to explain but it's like I hold it up without noticing and need to make an effort to relax it. Maybe it's scar tissue from the incision in the lip making it that way. The pro: I am still glad I did it because I think it looks a lot better and softer, not so square. The only thing I wish is that I had done more to be honest, like an under the chin incision so tissue could be removed as well because the tissue will not wrap tight on the bone. It's somewhat noticeable from a side view, at least to me, like a bit of tissue that hangs off the bone at the tip. This is one of those surgeries where you trade something for another in a lot of cases so you just have to decide if it's worth it to you. In the end I still think I look better.
