Disappointed - don't feel like the issues I wanted were resolved

Disappointed.

I had a rhinoplasty with Mr Ali just over a year ago now. Overall, while Mr Ali was kind and clearly takes time with his patients, I'm disappointed with the outcome: I don't feel like either of the issues I wanted resolved were resolved, and I will now have to have a revision rhinoplasty at my own expense to fix it, despite what it says on Mr Ali's website.

I've added my inspiration picture I showed Mr Ali, pictures of my nose before the procedure, straight after the cast came off, and now, a year later.

When the cast initially came off, although it didn't have everything I wanted, I was happy - it was straight and didn't stoop when I smiled.

However, within a couple of months, I noticed the tip dropping and a rounding in my nose. I raised these concerns with Mr Ali many times, but he insisted it was swelling at that it would go down.

However, right now it once again looks like a pre nose job nose. It's marginally better in some areas, but arguably worse in others.

I'm very disappointed with it and will need to get a revision rhinoplasty to correct it. unfortunately, despite what Mr Ali's website says, he advised me that this wasn't covered under the initial fee.

I'll detail my concerns and overall experience below, both the negative - but also the positive.

My concerns with my initial nose:

there were two main things I wanted changing about my nose:

1) the hook

2) the droop when I smiled

Overall outcome of the nose:

Both of these things are clearly visible in the before pictures, however, they're also visible in the after. While the dorsal hump was removed, the hook has now been moved down my nose instead of being removed completely. Further, the drooping experienced when smiling has not been changed at all, if maybe worsened.

This is the most upsetting thing. The major reason I got a nose job was because I hated the way I looked when I smiled (especially my side profile looked). To the extent that I would get myself to stop smiling by reminding myself how ugly I looked. I can remember being called a witch when I was around 11, going home and looking at my side profile, and genuinely crying for hours. As I grew up, it wasn't a debilitating insecurity, but it just wasn't *me*. I'd see myself on videos and I'd get shocked at my nose every time -- it wasn't how I saw myself in my head. I got a nose job to align what I saw in the mirror with what I saw in my head and finally be able to smile without stopping myself. When I first got the cast off, I finally felt that freedom. Now, however, I'm right back where I started.

As well as the issues from the side, I seem to noticeably look worse from the front - my nose is now fatter and is more swollen on one side. This is both clear from my front view photo, as well as from the photo underneath.

Mr Ali eventually acknowledged the issues, however not after first insisting that my nose was better than it was before, and implying that issues that I saw were in my head.

However, despite the fact that he acknowledged the issues (and others I didn't point out to him, but also noticed, like the fact that my nose pulls down when I speak) he informed me that he could do a revision to correct it, but it would be at my own cost. This surprised me, since part of the reason that I chose Mr Ali was that he guaranteed that any revision rhinoplasty within one year of the initial surgery would be covered under the cost of the first rhinoplasty, should there be an issue or unhappiness. This is currently even stated on Mr Ali's website (as of 24 Aug 2022). This is partly my fault. While I asked the nurse for the contract for this guarantee, she assured me not to worry because Mr Ali likes patients to be happy, and I took both her, and Mr Ali via his website, in good faith. I'm therefore also disappointed in myself in this experience.

As of my final meeting with him around two months ago, Mr Ali told me that the reason that my nose had rounded was because there was a lot of skin in that area (not swelling, as had last been told), and that it was because he had shaved down the area in surgery, leaving excess skin. However, this doesn't really make sense considering the fact that the area where the most bone was shaved down is not the same area that's rounded. Additionally, this issue should have been predictable and preventable.

The experience-- the good:

While I dislike my nose and will need to have it fixed, Mr Ali himself was a kind man. He took time to listen, insisted on spending more time with me when he felt like I might need more clarification (to no additional expense) and was personable. The nurses and staff at the hospital were also very kind and warm.

The experience: the bad:

Waiting times to get into the appointment were atrocious - no less than 40 minutes past the time agreed upon. This is probably a consequence of Mr Ali spending a lot of time with each patient, which is great, but it's just a word of caution.

There was also a time where I emailed Mr Ali and he did not get back to me. Although considering the time he spends with his patients, this is more minor.

Overall:

Mr Ali is a very kind man who goes out of his way to spend time with his patients and make sure they're comfortable - to the extent where I feel bad about posting this, because I don't want to hurt him. However, I'm really not happy with the outcome of my rhinoplasty; I feel mislead by the claim on his website of revision being included within one year; and will ultimately unfortunately have to seek a revision with another doctor at my own cost.
