My consultation with him went extremely well and it was one of the most fruitful consults I’ve had so far. He was very honest and realistic. Not to mention, I did not feel rushed at all!

My goals are to:

1. Get my slope back after my surgeon added a piece of cadaver rib cartilage to my bridge which makes it long and stiff
2. Get rid of the stiffness because of the septal and caudal extension graft my surgeon added to my tip Dr Murray said that he can either get rid of the dorsal graft or shave it down to make it smaller.

Since my surgeon also added grafts to my tip, removing the dorsal graft may or may not restore my nose back to its original form. He said he will judge whether it will look good after removing the entire dorsal graft in the OR. If it doesn’t, he will shave it down in order to fit the other tip grafts so that my nose won’t look strange. I think these are very clever strategies!

He also mentioned that he wouldn’t remove the caudal extension graft on the tip because it is great for support as it sticks to the septum graft for support. The disadvantage for it is that it is stiff but since I am ethnic, my lower lateral cartilages are weak so a cs (columella strut) graft might not be an ideal graft for support. He said that cs grafts dont work well for ethnic patients and he has had experience with them before and said that septal extension grafts are better for support. He also mentioned he prefers using a solid rib graft over DCF because he can control the rib better.

I also asked him about MTF cartilage and he and other surgeons have experienced resorption and although the rate is low, there is a chance of resorption in both cadaver and auto autologous cartilage due to the processing of the cartilage as protein can degrade when you thaw the cartilage. He prefers patients’ own rib as it is stronger and less chance of warping. He brought up a lot of great ideas and I think he is extremely honest, ethical and NOT money hungry (very rare in this PS industry). He told me not to rush as there is still a lot of swelling in my nose. I highly recommend you to get a consultation with him as he is knowledgable and honest.
