Quaternary Rhinoplasty Miracle (W/rib Graft)

I came to Dr. Rohrich desperately seeking repair of a pollybeak (as well as excessive deprojection, foreshortened nose, and alar base scarring) resulting from a tertiary rhinoplasty with someone terrible, who, in retrospect, I did not sufficiently vet. (See my other review.) I no longer looked like myself - I looked much, much worse. At least two surgeons I initially consulted told me that these problems (esp the pollybeak) could not be corrected or would possibly recur.

I found Dr. Rohrich after researching articles on pollybeak correction and secondary revision rhinoplasties generally, many of which he authored, and in a rare (and short) list of best rhinoplasty surgeons posted somewhere on this site by another surgeon. I also interviewed a handful of other surgeons who have authored publications on revision rhinoplasty. Dr. Rohrich explained that I could never "go back" to my old nose, but was confident that he could make some serious improvement. (Side note: he also recommended an excellent plastic/maxillofacial surgeon (Dr. Mark Urata) to revise a disastrous sliding genioplasty. Incidentally, when I saw Dr. Urata for a follow-up after Dr. Rohrich redid my nose, he remarked, "I didn't remember your nose looking this good," and when I said it was thanks to Dr. Rohrich, he said, "Oh, he's one of the best in the country. One of the best in the world, actually.")

Ultimately, I road-tripped in March 2021 to Dallas - 15+ hours in the car with my husband, mother-in-law, and toddler. It was expensive and a huge headache, but one of the best decisions I've ever made. When the splint came off (around day 5 or 7), I was so shocked, relieved, and ecstatic that I actually cried (both in the office and at random moments for the next several days. My mom said, "You look like yourself again!"

With Dr. Rohrich's blessing (and given the constant masking anyway), I wore tape 24/7 for the first 13 months post-surgery. Now I tape only at night and plan to keep doing so for awhile longer. I've also gone back to Dallas regularly for kenalog injections to prevent the build-up of scar tissue.

This post has been a long time coming because (1) I wanted to wait to make sure it still looked good after it had substantially healed (esp to see if the pollybeak would recur) and (2) I wanted to post before and after pics, but it's taken me this long to even be able to look at my before pics. I am now 15 months post-surgery. At my most recent appointment, Dr. R indicated that my swelling reduction was ahead of schedule but that I still have 12-18 mo to go before I will see the final results. My only regret is not going to him in the first place - I could have saved myself an inordinate amount of $$$ and years of emotional agony. I have had 5 noses (including the one I was born with) and this one is hands-down the best.
