I decided that I would wait one year post surgery to write my review. I was very impressed by Dr. Ackerman during my first visit. His staff is second to none. The pre-op visit was very comprehensive and the doctor was direct but kind and extremely reassuring that I would get a "wonderful result." Post-op/follow-up care was comprehensive and encouraging. I appreciated the thoroughness of Dr. Ackerman and his staff. I would also like to say that special attention was give pre-operatively and the day of surgery to address my diabetes, which was reassuring to me. I was thrilled with my results for several months. So now, one year later, I am prepared to dive into the "BUTS;" the reasons I am extremely disappointed with my results. First of all, I understand that everyone's results are different, but in all honesty, I expected my results to at least last a year, for goodness sake. At my four or five month post op visit, I mentioned to the doctor that I was concerned about the deep wrinkles that were reappearing on my left cheek. His response (and please note that I am paraphrasing, not quoting) "I did the best job with what I had to work with." Wow, how unflattering a comment. He did suggest that he would give me a discount on a full laser treatment to help these magically reappearing wrinkles. I'd just been through major facial surgery (face/neck lift, upper bleph, and partial direct brow lift), and was mortified at the thought of getting my face burned off (I'm trying to have a sense of humor here). Nonetheless, his staff provided me with a quote of $3,500 for the laser procedure. Wow, let's add that to the $18,000 I had just spent four months earlier. He also was relentless about me getting Botox for the frown lines between my brows. I did the Botox. Christine, his RN, is simply beyond awesome. What a terrific asset to this office. I showed Christine the excess skin still apparent in the upper eyelid area of my left eye. She was very candid with me and said the doctor should have taken another 20% from this area. I had the Botox and it helped tremendously with lifting the eyelids, but after an upper bleph and partial indirect brow lift, I shouldn't have to get Botox every 3 months. So, to sum it all up, I feel like a walking/talking dichotomy, because my "experience" was wonderful, but my result is a disappointment every time I look in the mirror. My cheeks look today just like my "before" pictures, in terms of the wrinkles. I am going to consult another doctor across the parking lot from Dr. Ackerman for a revision or "fixing" all these reappearing wrinkles.

I have similar feelings about the 'mini-lift' I had 2 years ago. I was impressed with the doctor, had a great experience with the procedure, follow-up and staff. I was happy and hopeful during the first few months, when my face was still swollen. At the time, I didn't know that I still had swelling, because it doesn't appear to be swelling, it's just part of the result. After the swelling went down, I think at about 1 year, I saw some slight jowling and lines return. Definitely not as noticeable as before the surgery, but I was hoping not to see them at all! I also had a bad response from my doctor when I showed him that I have ONE pixie ear. He told me that I must have had this before the surgery. Really condescending. I could show him pictures of myself from childhood onward that would disprove him, but I doubt he'd do anything.
At this point, I don't trust any before and after pictures that don't show the patient at least a year out, when the swelling is gone.
Like you, I still feel I need botox, filler and maybe something else. I'm trying to relax and accept some of this aging process, but that's tough for me, and a lot of us who are drawn to this industry.

lower fl's do NOT address the tissues underlying the smas...without moving the muscles forget any long term affect

So, what you're saying is you actually needed a more aggressive procedure than the SMAS? So very sorry to hear. What a thorough and fair review. What doctor across the street did you see and did he help? Blessings.

a smas lift lasts about one year on a 50 something year old! If you are in your 40's they last longer. Few doctors want to take the time it takes to perform a real deep plane facelift. 4+ hours just for that..tack on more time for eye work if you need it or brow lift
