I am writing this review because I choose to go with this Dr after reading all of the positive reviews from this site. I however didn't have as positive of an experience as everyone else on here . I made a consultation for a breast augmentation after not doing too much research on breast implants themselves, I figured I would learn all I needed to know at the consult. My main concern was finding a credible Dr, in which it seemed I had. Upon arriving at my consult the front desk staff was very friendly and made me feel very comfortable. The lady that took me back was not quite as welcoming, she went over the basics and then took me to try some implant sizes, she offered me two sizes, the first I felt was pretty good the next I felt was too large. She wrote down the selection and brought me back to the room where I waited for the Dr. When the Dr came in he asked if I had gone over the packet that was sent out and the different choices of implants, I told him I knew I wanted silicone and the round not teardrop. He looked at the size I chose and said it would be good. He didn't explain much of anything other than tell me to make sure I choose a large enough implant since 99% of his redo's are because patients are not satisfied with their size. I decided to schedule a surgery date because I felt based off the reviews and after meeting with him that he knew what he was doing, and had lots of experience and would be a good choice. Surgery day came, I brought several pictures of breasts that I liked and did not like. He took a look at them and agreed. After surgery Dr Tholen didn't check on me, his nurse came and checked on me briefly, but I was so groggy I don't remember much. That evening I took my first look at the new girls and immediately noticed one was larger than the other, I was expecting they would be perfectly the same size since you pay so much money to have them look perfect. That night when the nurse called to check on me I asked if the same size implant was used in both breasts and she said yes. Now that would be fine if both breasts started out identical, but as every woman one of my breasts were slightly larger to begin with. I asked her why he would use the same size implant and she said she was pretty sure he did, but she didn't have the numbers in front of her, she also said unless the Dr and I discussed it beforehand he would have used the same size in both. (She was also pretty rude and wasn't comforting to my concern in the least). I was completely floored by this response , why should I have to tell the Dr to make sure they come out the same size, isn't that a given that someone getting a breast augmentation would want them to be the same size after surgery. I knew there was swelling and settling to do so I tried not to over react too much, but I was pretty upset. Fast forward to present day(months later) after I have given time for settling/dropping/fluffing I am not satisfied. I am much smaller than pictures I see of other women with similar stats that received the same cc's, my breasts are still different sizes not to mention my crease incision is higher than the actual breast crease especially on the smaller breast. After doing research I found out breast implants come in different "profiles" which drastically effect your final result(which would explain why the same amount of cc's in a low profile/moderate profile implant will look smaller in your body than the same cc's on someone who chose high profile implants). Profiles were never discussed with me at my consult nor was it mentioned anywhere on the website or in the pamphlets. Had I known there was a difference in profiles I would have chosen high profile implant to gain more projection and feel I would be much happier with my look. I would not recommend this surgeon as he did not provide all the information and guidance necessary to have the best outcome possible.

I also found out after my BA that there were choices I did not know about. Neither my surgeon or the lady who did my sizing spent time educating me. I had never seen an augmented breast until my own, so I did not know what to expect in either size or shape. I do plan to get a revision, but I don't know when or by who. Are you considering revision?

I am so sorry to hear you are disappointed with your results. I can understand your frustration in thinking your PS or assistant would educate you a bit more on implants. Did you have asymmetry pre op? Does your PS agree that your "wish photos" look nothing like your results? Is your PS willing to do a revision? Take care and keep us posted!

                        Hi i know exactly how you feel! my past ps never really educated me on profiles either! He put a 590cc ultra high gel implant in me and it looked like a B cup! I hated it! i had a revision done sine than I' m 3 and half months post op! I have 800cc high profile gels and i love them! my current ps is Dr.Schreider! I found him here on this site! He gave me beautiful results!

It's hard to respond objectively without sounding defensive to an unhappy patient who has so much anger she has gone to the trouble to do all this typing, yet didn't take the time to read the information provided to her prior to her consultation. 24 pages of information, including specific information about implant profile is in my Comprehensive Guide to Breast Augmentation (also printed here on RealSelf in my Articles section), explains and answers most if not all questions I have been asked by actual patients undergoing this procedure. For example, 300cc is the same volume regardless of implant profile, but high profile implants are narrower and do indeed project 1-1.5 cm more than moderate profile implants (about 1/3 to 5/8 inch). That projection is at the expense of width (narrower)--higher profile could have indeed been used, but this and any patient would end up being the same size (too small in this patient's case), but with wide "Grand Canyon" cleavage that almost every woman finds very unattractive. Too small is too small, and it is ironic that the one thing she did get from our consultation (much more than "didn't explain much of anything other than tell me to make sure I choose a large enough implant . . .") is the main dissatisfaction here. Of course I talked to her right after surgery; however, very few patients recall anything I say prior to my next surgery as they are just out of surgery, and still feeling the effects of residual anesthesia/medications. They wear off rapidly in the next hour, but I am already in the operating room with my next patient. That is why I see virtually all of my patients the next day. I am truly sorry this patient feels that too little information was provided; my nursing staff feel I explain too much and that much of it is forgotten. But that is why every patient gets detailed written information and is offered unlimited free consultations to answer any and all questions. Lest I sound defensive by more discussion about the details of what she writes, I would invite this patient to return at any time and I promise to take all the time necessary to review her result and see what can be done to correct any concerns she has. Every patient is offered this; everyone absorbs information differently and I strive to make sure this type of unhappiness occurs as infrequently as possible. Even if 1% of my patients is dissatisfied, that is still half a dozen per year (and several hundred happy ones). Please call; my time is yours. Dr. Tholen
