Is it possible to see a head on shot as I am interested in alar base reduction as well to narrow my nostrils. Are the alar base incisions well hidden?
Well, like you can read almost everywhere here on realself, the scarring is individual from person to person.. I would imagine that someone can have highly visible scars even though the surgeon did a great job, because we all heal differently. Since it has only been 2 weeks after surgery they ARE visible, but only if you look close.. He only cut right under the nose, and not around the sides of the nostrils. I can upload some pictures.
Oh! One important detail. One of the alar base stitches went out some days after surgery, unfortunately. It could have been me using the q-tip too hard, or just the tissue "pushing" it out. It shows, but I refuse to get stressed out about it. Prof. Palma said that it would hardly be visible after it has healed. If it bothers me too much it can probably get fixed later.. Maybe make new incisions on the sides and put one suture in. (If any plastic surgeons are reading this, feel free to comment!)
I love your results. I have a simular nose and I have to have an alar base reduction too and I am afraid of the scars. Can you show me a closer picture of the scars? Do you think people will see the scars if they are close?

I also just want to say that if the incisions are directly under your nose, and not in the creases on the sides, they will be on a place where hardly anyone looks. What made me decide to do alar base reduction was this reasoning: Scars can be treated with steroid injections, laser treatment etc. There is not that much to be done about flaring or big nostrils smack in the middle of your face.. except revision surgery. One of my sutures in the opening of the nostrils came out a couple of days after surgery, giving me a small gap in the base of the opening, and STILL I don't regret it. Leading up to surgery I have been mentally preparing myself not to get hung up on whatever asymmetry or small little thing that could go wrong - and I am glad I did. i did the surgery so that I could continue on with my life NOT thinking about my nose all day everyday. I will see what my surgeon says about the gap on my 3-4 months post-op control, and update then!
Thank you for your answer, my surgeon wants to make the incisions on the creases on the sides, I am afraid of scarring badly and have a beautiful nose but 2 horrible scars just in the middle of the face! I am looking for pictures with scars in that area to get me an idea of how do they usually look.
I am glad to hear that just 2 weeks postop your scars are visible only if you look close

Thank you very much for your help. Hopefully the gap you have heal nicely and you dont need any more treatments.
I hope you land on a decision that you are comfortable with, and offcourse that you will love your new nose :) Best of luck.
Hello! Har du någon bild på resultatet? Dem verkar ha försvunnit men skulle så gärna vilja se då jag funderar på operation med prof Palma :)
Thanks for the information egw88. I think you are healing beautifully and your nose is very cute. Your doctor did a great job.
ORIGINAL POST
I have never liked my nose. I always thought it...
I have never liked my nose. I always thought it was too big and that the tip was bulbous and masculine. I also had long nostrils. On two different consultations in my home country (Norway) I was told that the result would not be flattering. I gave up for a couple of years, but finally decided that I would give it one more try. I found Pietro Palma online and was off to Milano for a consultation a month later.
He truly is an artist. He had a clear vision of what he would have to do to give me the result I wanted. Still, he was careful to point out what could go wrong in the surgery, especially regarding scarring from the alar base narrowing I wanted (as big nostrils was my worst fear). I do not remember all the medical terms, but the operation was extensive: He gave me a radix graft, he deprojected the tip, he took the tip back and made it smaller and narrower, and reduced my nostrils. It is now 8 days since I had the surgery done, and I got to take my cast off yesterday. I have no bruising and the swelling of the nose is hard to spot if you didn't know that I had rhinoplasty.
This might sound corny, but I feel as though 10 kg has been lifted from my shoulders with my new nose. The nostrils are still a little bit big due to swelling (I think), but there is no denying that it is pretty! I like the fact that I still look like me. Me and Prof. Palma decided that I should keep my tip somewhat rounded, as it is what makes me cute. The photos are, as said, 8 days post-op, so still early, and the tip is swollen.

Looks lovely! I'm so glad you are pleased. Thank you for sharing your experience on RealSelf.
Wow looks great! Im from sweden and i was just wondering just about how much the entire thing costed? Like trip+surgery
Hei! Du kan lese svaret jeg gav til "loverealself" over her. Der står alt :)
I`ll write in english in case someone else wants to know. 8300 euros for operation etc.. Then about 1100 euros to stay 10 days after operation.. And all flight tickets back and forth (consultation, operation, post-op control) I don't remember. When you add it all up it offcourse costs alot. But this was what I had to do to get this done, feeling that the norwegian surgeons I visited wasn't good enough. I didn't go to Oslo (the capital in Norway) but it costs the same to travel inside Norway as it does to travel to another country :D
You look great. I'm from Europe too and I'm interested in knowing price as well. Sofar I've only come across skilled surgeons in the US.
How long time did you spend in Milano ? Did you check into a hotel and did the Dr. have a nurse to check up on you during your stay? Xx thanks

Hi! Thank you! Fees for Palma, his surgical assistant, the anesthesiologist, and the private hospital that the surgery was performed at (La Madoninna) combined was about 8300 euros. I saved for a long time, because I wanted the best. Also, my specific case was quite extensive. The price was calculated after we decided what would be done (There was no "fixed price" on his website). Besides the first time consultation the fee entailes a 28 hour stay at the hospital after the surgery, and all post-op controls with Prof. Palma. I am going back in january/february (3-4 months after surgery) for my first check-up. Since I live in Norway I will probably not come to all the check-ups. I have read on different reviews here that some surgeons have one or none follow-ups afterwards. This is definately not the case for Prof. Palma. He came to see me the morning after surgery before I left. I rented an apartment right next to the hospital though airbnb, and my mother stayed with me there for 10 days. I got Prof. Palmas cellphone number to call anytime I wanted, but when I had questions I just e-mailed his office (with quick response every time!).
I arrived in Milano 2 days before surgery. On the 7th day after the surgery I came to Palmas clinic and he removed the cast and the silicone sheets inside my nose. I stayed in Milan for 3 more days, and returned to Norway on the 10th day (the written information I got said that I should wait 2 weeks after surgery to fly, but they told me it was okay to fly after 10 days).
First of all a huge thank you for answering me so thoroughly and taking time to do so. I am actually so happy I came across your review in the first place since I more or less had decided to go with a doctor in LA but due to the planning around family and business, I hesitated. This particular doctor had 9 months waiting and this seemed too far for me to plan around giving my private/business traveling being based in Europe. But he was THE only one with consistent fantastic results so I had more or less gotten use to the idea of traveling that far.
So I am soo excited and GRATEFUL for your review and to have found a doctor on European grounds who seems so capable. And from your photos I can tell he did an excellent job.
I will send Dr. Palma an email explaining my case.
Thanks a lot and I wish you a further happy healing XX

Oh, just another thing I thought of. Did Dr. Palma and you decide upon your procedure, look of your nose after operation, during the first consultation? The reason I ask is because on his site he suggest two consultations before deciding. But since we're both out of town patients I would love to decide on everything during the first consultation and stay in Milan for only 1 day. Thank you
I only had to come to Milan one time for the first consultation :) He took pictures of me there and later sent me simulations by e-mail of different potential outcomes. I commented on the pictures and how I wanted it to look, and then the next time I saw him was before the surgery, where we talked through what we had decided to do one more time. So happy I could help! Best of luck :D
Thank you so far for all your help. I hope you wil post more photos with time XX
Looks beautiful, he truly is an artist, no bruising and he did all that, just wow
Thank you!
Great results. I particularly like the angle between your lip and nose now. I see Dr Palma seems to pay a lot of attention on that area as he even makes some of his patients pull their lips together with their mouths to show the angle (on his website pictures). I was so sure about going to USA for the op but I think I will go with Dr Palma since he seems to be one of the experts in Europe. How did you come to deciding on Dr Palma could I ask?
