Here are some more pics. There is scar tissue buildup inside on both sides. Much more on the left than the right. It is significant enough that it affects the air flow when I breathe but I've learned to deal with it. Before this revision I talked to the doc about removing some of the fatty tissue that makes my nose look so bulbous in the tip, to shave down the bump, and to narrow the bridge some. He didn't do any of these things.

Thanks for posting close up pics. The breathing problems alone should justify another revision with an ENT who performs a lot of "ethnic" rhinoplasties. Did either doctor place grafts in your nose during surgery? I ask because in bulbous tips, the usual MO is to harvest the patient's own cartilage (septal or cochlear) to lend support and more definition to the tip, rather than simply "de-fatting" it. Since you now have functional problems (restricted breathing), all or most of your next revision may be covered under insurance. It's something you can look into.
During the second surgery the doctor did put some tissue into the tip. But I was led to believe that the procedure would be a combination of defatting as well as reshaping that would result in a reduction of the bulbous appearance, not that it would be just inserting cartilage.

I definitely need to fix the problem with the scar tissue because its actually pretty bad. But I'm nervous that more surgery at this point may actually make it worse. Need to find a surgeon who has a lot of experience with these types of revisions.

Hey FLTDUDE! Sorry you're dissatisfied with your results. I've been through a few surgeries so I can relate, but there's definitely hope. You stated that your doctor didn't do several things. So what exactly did the revision entail? Are you still communicating with him? Did he offer any explanation about why he didn't address your specific requests?

If you are searching for another doctor, try contacting Dr. Le in Baltimore, MD. He is an ENT/Plastic surgeon; so he will address the functional and cosmetic issues. He does a good job de-fatting bulbous noses. You're not quite at the end of the healing process, so you will likely have to wait a bit longer, but if you're willing to travel Dr. Le is a good bet. Stay positive and keep us posted!
No, I am no longer communicating with that surgeon. My last post-op follow up with him was in in March. Each time I went for a follow-up visit he would tell me the same thing, that the swelling will go down and that the nose will look more refined when it does. He claimed to have done extensive work to repair the damage done by the first surgery, including removal of scar tissue and repair of cartilage on the left. He said that after he went in, he found that the upper and lower left cartilage were completely missing and that's one reason why my nose was misshapen after the first surgery.

Okay, I see. Did he offer you steroid injections to help control the reformation of excessive scar tissue? There are pros and cons about the injections and I'm not certain if it's 100% effective for everyone, plus it probably won't solve all of your issues, but it may be worth a try. I think they are most effective if given during the first year of surgery. Good luck!
I received a steroid injection from him only once. He didn't seem too willing to do it. He did explain that he isn't a fan of steroid injections. Perhaps I should contact him again about that.
Your nose looks great. Don't go for drastic changes because that's how you end up looking like Latoya Jackson. As the time goes by you'll see that the tip is going to become more and more refined and less bulbous.

Definitely don't want the Latoya Jackson look. I met with a surgeon about doing another revision. He told me that I still have a lot of swelling inside my nose and that I should wait a while longer before thinking about doing more surgery. He feels that the tip will look better when the swelling is completely resolved. He said that because the second surgery was so extensive I should give it at least a full year and a half instead of the usual 1 year period before considering a revision.

This is my second nose job. The first one was a calamitous experience. Went back years later to try it again. My nose looked more like a snout. The tip had no projection. The bridge was too wide, I had wide, flaring nostrils, and a lot of alar fat. At first I liked this revision, because the AFTER is a big improvement over the BEFORE. I was told that it takes at least a year and sometimes longer for the nose to take its final shape. But I can already tell that much of what's left inside isn't just swelling but scar tissue that will remain---lot's of it. I also think that my tip is still too bulbous, that I still have too much alar fat, a too wide columella, and a hump on the bridge that I had hoped would have been removed. Overall, I think that the doctor did a good job, but the more I look at this nose the more I'm not liking it. I mean, I'm not going for the extreme 'I had a nose job' look, but still.

This was my second nose job, and I am scared to do it again. But it bugs me enough that I'm planning to go back under the knife again next year. I'd especially like to hear from other docs on here who do these surgeries.
