I took some pictures today, to show what it looks like post op. I also updated the cost, so I'll address that first. The original quote I got for just the jaw contouring was $4,725, including anesthesia and operating room fees. I added the undereye bag removal later, so the final total for both was about $8,000.
Overall, I'm very happy with the results. As you can see from my pictures, one side looks great and one side has some extra cheek fat/pre-jowls. However, that one side was already like that before I had anything done, so it doesn't have anything to do with the procedure or the doctor. If you look at one of my pre-op pictures in the pink dress, you can see it on the right hand side. I took that one in a mirror, so it's a mirror image and is flipped. Post op, you can see the same thing on the left hand side. I took a couple of pictures lifting the fat up so you can see the jawline underneath. Dr. Mardirossian's work is excellent. Now I just need to figure out what I'll do about the extra bottom cheek fat. I feel like I'm a little young for a facelift, so maybe I'll try some fillers to even out the bottom or in my cheeks to lift it a little. We'll see :)

I just got a medical review/report by a craniofacial surgeons(to submit to court.) It states clearly that with jaw angle reduction, once the jaw angles are cut, the soft tissues get detached and move forward which results in jowling and this is a common consequence of jaw angle reduction.The periosteum membrane that covers the jaw angles gets detached. And this is considered " NOT STANDARD". To slim the jaw, it should be sagittal split plane which preserves the jaw angles to support the soft tissues( plus the periosteum membrane does not get detached?). But reducing the jaw angle takes away the support for the soft tissues and causes sagging. So " jowling is a common consequence of jaw angle reduction". I will have to have my premature jowls(soft tissues)undergo further reconstructive surgery,some doctors call it a SMAS lower face lift. We plan on making a very detailed youtube vdo made to explain all the information given by craniofacial surgeons regarding jaw angle reduction, many doctors dont inform people about this traditional jaw angle amputation surgery and how the soft tissues get detached and move forward resulting in premature jowling.

The informative video would be so great to see. I noticed some very minor jowling using botox to reduce the jawline and attributed it to the Botox so have been scared to touch the bone and underlying tissue. I consulted with a Beverley hills plastic surgeon who refused to touch my jaw because it would mess too much with the other angles in my face (I have prominent cheekbones and was concerned about having an upside down triangle face). He was okay with doing a chin reduction . While the other Beverley hills plastic surgeon said he'd do jaw and chin reduction and I asked him what would happen to the soft tissue and didn't get a clear answer. I don't know which is the best way forward to feminize my face. It's so hard to choose a facial procedure where millimetres count and if over or under done the overall result is compromised.

GG567 I'm in the same boat. Have you made any decision or gathered further info on this? I consulted with a few surgeons in Beverly Hills and they all advised against jaw reduction. At the most they said a little fat or fillers on other parts of the face to soften/ feminise it. The doctors in Seoul however are very comfortable performing jaw surgery. I hear too many horror stories about SK plastic surgery so I'm wary. Check out beautyme2015 on realself and Dr. Yoo from Pitangui in Seoul.

Dr Yaremchuck has watned the public against face bone reduction,it causes loss of support for soft tissues/skin and premature aging.He's been having to see patients with collapsed faces and some cases are irrepairable.Face bone reduction will give further problems costing a fortune to fix.BUt not always repairable.Doctors know it causes problems/premature aging but some still doing this surgery on people.

A surgeon I consulted with in LA wanted to do an external incision to shave down my jaw, and lift the soft tissue as I asked about potential jowling afterward. Would this be an effective way to address both issues with one operation?

I know at least 5 people on line with disastrous problems from face bone reduction. Please email Dr Yaremchuck, he's a craniofacial surgeon who has had to meet so many patients after face bone reduction. He's stated that face bone reduction is not wise because it will result in excess soft tissues/sagging.There will be a visible scar w/ external incision.Please email Yaremchuck with pictures,he is most knowledgeable regarding this and he's been honest to patients.Always get 2nd and 3rd opinion from specialists.Dont do anything that will result in disaster and endless amount of money to fix, some cases are not repairable. Email Yaremchuck and see what he says. Any face bone reduction will cause premature aging,sooner or later and that's a fact.Google " face bone loss and premature aging" and click on IMAGES. , you will see how bone loss makes the face collapse and sag as can be seen when people get older.

I googled those keywords and found a lot of pictures, but almost all talk about teeth loss which results in aging. I think the key is, when cutting bones, don't overdo it. It goes with any facial bones including teeth.

The point there about teeth loss is that loss of teeth will contribute to bone loss of the face bone, And all doctors know this and the fact that any face bone loss will take away the structure and support for the facial soft tissues and skin. So facial bone loss contributes to aging that's why face bone reduction is not a wise thing, as Michael Yaremchuck has posted in realself.
When the doctor has to access the facial bones to cut or reduce the bone, the doctor has to strip off the periosteum membrane(muscle) that covers the bone. Once this muscle has been detached off the bone, the muscle will move forward and out of place; so face bone reduction will have direct and immediate impact on the muscle/soft tissues.

Generalizing. Some people need their facial bones reduced. Yes, afterwards there may be sagging, which is not irreparable.

Just over two weeks post-op. The swelling is going down and I can chew lightly, but there are a couple of areas that are still a bit sore. The stitches haven't dissolved completely so I'm not fully brushing my bottom teeth yet. Just the back of my teeth, where nothing happened.
I still have dark circles under my eyes, but my expectation was that they wouldn't really go away since I've had them since my teens; my skin is just really thin there which is what causes them. The bags seem to have significantly decreased though.
I know the final results won't be apparent for a month or two, if not longer, but so far it's looking good!

Hi :) did they file your jaw? Or what was the procedure? I've been seeking for other options since they had said it was a very risky surgery. But I dislike my square jaw :(

Hi, yes, the back and sides were filed. I didn't think it was particularly risky, where did you hear that? My entire face was flat on the sides, as you can see in my first picture. Contouring my jaw was the only way for me to get a better shape overall.
I have a cold at the moment, but I'll post an updated picture in a week or so, which will be about a month post-op.

Things to consider regarding jaw bone reduction surgery:
1. A surgeon can over cut your jaw angles resulting in indented jaw angles which will require reconstructive surgery, such as wearing jaw angle implants and screws in your face.
2. A surgeon can perform a crooked cut on the mandible body, requiring reconstructive surgery which your mandible body will have to be burred down to bring back a normal straight jaw line.
3. To access the jaw bone, they have to strip away the soft tissues that was attached to your jaw bone. Once this soft tissue is detached, it will move forward. This is a common consequence which results in sagging at the jowl area. Premature aging.
4. Any bone reduction on the face has an immediate impact on the soft tissues.
5. One can end up with permanent nerve dammage because the major nerve site runs from along side the chin area all the way to the jaw angle.
6. People have ended up with broken jaws.
7. People have ended up with their teeth/roots affected.

I'm out of town visiting friends, and the good news is they don't know that I had the jaw contouring done. (I did tell them about the lower bleph) I could tell they thought something was different, but that maybe they thought I gained weight and was showing it in my face. I'm eating regular food now, but still try to stick to softer foods because the stitches haven't dissolved yet and chewing can be a little painful. For non-soft foods, I just take tiny bites and chew slowly and carefully, mainly with my front teeth. I still have a fair amount of swelling and soreness. I'll post more pictures soon.

where are the incisions for your jaw? I'm surprised that is a procedure that can be done in office.

The nurses told me to avoid warm/hot/ spicey foods which contribute to swelling. So for 3 wks, I lived on milk, juice, and soups that really cooled down. I craved salt so had to have soup and super soft processed cheese. And no sugar, the bacteria in your mouth multiplies and can contribute to infection.Had honey. Mouth washes, gargled and washed mouth with salt water. Small baby tooth brushes.

I didn't really think about the sugar thing until you mentioned it but it makes sense so I've definitely been avoiding it, most of the time. If there's something I had to have, I just made sure I brushed and gargled well. Thanks :)

This tells me he is well aware of the potential sagging. Did he discuss the face lift need with you? But since you are only 38, he might have decided you don't need it. Either way, if you do decide on getting lift to address the jowl, I would go back to your ps if I were you. But then you might not need it at all.
