I did 2 INFINI RF + Fraxel CO2 last year and finally got my pictures from the doctor with all parameters

Infini 1st pass level 3 Pulse Time 300 msec depth 3.5 mm
2nd pass level 3 pulse time 300 msec depth 2.5 mm
3rd pass level 3 pulse time 200 msec depth 1.5 mm
then right after Fractional CO2 beam size 120 um peak power 30 watt pulse energy 20 mJ

I'm a little bit discouraging as I don't see a lot improvements - even with the little improvements there I was wondering if that's from INFINI or Laser.
I consulted a few more doctors, most of them still say INFINI would be the best for my scar types and I should continue that route.
1. suggested subcision right before INFINI x 2 session (but i don't think I have much tethered scarring so not quite sure about that)
2. suggested INFINI x2 session + BellaFill (my concern is the filler would make my face puffy? or unnecessary increased volume, bumps, etc??)
3. suggested INFINI + PRP (also I am questionable about how much PRP can do)
some other suggestions like micro-needling (def less powerful than INFINI so I passed on that) and erbium laser (seems older tech than INFINI).
Really want to get some advice from experts (esp. INFINI) -
- should I continue?
- anything (parameter) setting were wrong in my previous 2 treatments?
- do I need filler as the final step any side effect/impact?

why is your doctor only using level 3? dr davin Lim uses level 20.. you should ask him or look him up on youtube. his results are amazing!

i think that he would recommend tca cross for your boxcar scars and then infini at a higher level. maybe a little filler but its hard to tell with the lighting of your pics

Don't get discouraged. There is new setting protocol for infini acne scar treatment. Level 3 is best. I would look into TCA CROSS first.... then go from there. You are on the right path!

Thanks I saw Dr Weiner's comment on your post. My current doctor doesn't recommend TCA CROSS , is that an obsolete way compared to INFINI?

Dr. Lim's settings in his videos are outdated. We have done research and found that settings too high lead are counterproductive and lead to smaller coagulation zones. The optimal settings use lower treatment levels and longer pulse durations. On US devices, there is now an interface that breaks down the energies to "Low, Medium, and High" and accounts for the new longer pulse durations. Deeper needle depths can be treated at higher levels than more superficial treatments. The low setting is still very powerful. Your deeper settings were comparable to medium. Please remember that the higher the settings, the more prolonged redness and swelling is possible, the risk of healing issues (if the treatment isn't done exactly right), and pigmentation issues. The most superficial pulses in asian skin should never be above low. Staying 1.5 or deeper is needed as well. I would continue with Infini. I don't think filling those scars is possible (they look mostly like boxcar and icepick). PRP might aid the healing but can't be sure if it improves overall outcomes. Dr. Lim uses PRP on many patients. After my presentation in Australia this summer, he has converted all his laser treatments to Infini for acne scars. I don't see any other technologies that match the Infini results for acne scars. TCA cross will possibly help, subcision will not help much.

Hello Dr. Weiner, from Dr. Emer's responses to Q/A postings, I can tell that he is a fan of microneedling with PRP. How is this compared to Infinite RF? Are there conditions (skin type, scar type) that regular microneedling may be more ideal than Infinite RF? Thanks!

The Infini is a much stronger/aggressive treatment. PRP can be used with the Infini. I apply a mask with PRP for those which desire it.

I had three infini treatments last year for wrinkles and skin tightening at Dr Sudins office in McLean Virginia. A technician did it and I saw 0 results after spending $4000. Do you think the wrong settings were used?

It is hard to say. The settings are preprogrammed now as high, medium, and low. If you are a smoker or have a medical condition in which collagen will not be as stimulated, your results will be less.

I am skin type 2 with acne scars. Went to a nurse in NY who doesnt have the new interface. She asked me for settings. what are these medium and low settings for Cheeks and temples?
my understanding is medium is TL5, 400ms?
What is 'low' settting?

It is actually proprietary, so I don't know the exact conversions. I think low is about 3/250-300ms.

These levels are not currently the best after further research. Too high, and the energy doesn't travel through the tissue. Better settings are lower and longer pulse durations.

I wouldn't say obsolete. Seems to be an affordable treatment with proven results, if you ask me. I think it works on deeper ice pic scars. Not that Infini doesn't work....just TCA CROSS might do better on ice pic scars. Might be a good combination treatment after/before Infini!
