Grafts Coming Down Around Inside of Alar Rim

I had my third septo-rhinoplasty 6 months ago.This was a tweak to fix my dropped tip and improve my breathing as tip and breathing had deteriorated after 8 weeks of having my second rhinoplasty.
After third rhinoplasty I was very happy with my nose until I noticed some changes, and most recently the same change has increased.
The problem is very difficult to show using a standard camera. And all the photos I took were using a standard camera.
I am not sure what this area of the nose is actually called, it’s like the area above the nostril, below the tip and next to the columella, inside part of the alar rim/infratip lobule / facet.
Basically I asked my surgeon to make my nose more defined and he did, the area that I mentioned above had more of a contour, like dents and that gave my tip a nicer appearance.
This started changing after post op 3 months and I raised my concern to my surgeon. He told me it is ok and to leave it alone. I thought it was scar tissue as I had that issue after my second rhinoplasty but my surgeon told me it is the grafts he put in my nose to hold my tip up, and the grafts have come down a little and he felt it looks fine. I bit my tongue and let it be and was just hoping it does not get worse. I saw my surgeon again for a follow up and raised my concern about this area. Again he told me to leave it alone and that it is fine.
I do believe that my surgeon has tried with my nose but this area which has changed really it’s a small area but makes a huge difference to my nose. A few weeks ago I noticed that the grafts have come down more and now there is no tip definition left at all in that area. It makes me sad to be honest I spend so much money to fix my nose, and although my tip rotation, tip size is fine and breathing is better I really wish that this small area could be fixed without opening the whole of my nose.
Can this be done to fix a small area? I may have to persuade my surgeon although he thinks I am very fussy, I don’t think so I mean I do have an asymmetrical bridge and other parts of my nose are not perfect but I have accepted that and am not complaining about it, he had two attempts at fixing my bridge and first time it was not fixed at all and second time only fixed somewhat. We are going to use fillers now to disguise that area.
I want advice on how I can go about fixing the area with the same surgeon? Does anyone have any suggestions or experienced this? If so what did you do about it?
Don’t get me wrong my surgeon is a lovely person and has improved my nose but just this small fix will make me so much happier like I felt before the grafts came down.

will tell you later. I have been able to communicate with him whenever required. I had my open revision rhinoplasty in 2014 and 2015 and I am happy with my results. I have been meaning to write a review for months but had not got around to it until now. I had a first rhinoplasty before I went to Mr Matti and suffered trauma to my nose after, my first rhinoplasty was on the NHS and the results were bad he left my nose very weak. Every part of my nose had something wrong with it which needed addressing. Including a narrow bridge, over resected, deviated bridge bulbous tip, deviated septum, enlarged turbinates, droppy tip, depressions on both sides of the nose, large nostrils because of a hanging columella. I felt my very first consultation with Mr Matti was a little rushed but all the other following consultations I had with him he let me take my time and I had about 4 consultations before he actually done my nose. I felt he is a lovely man and is very fatherly like, some people may not like that but I don't mind. I realised quickly that he has a good eye for aesthetics, he knows what would compliments the rest of the face and features and my nose is very close to the morph he done originally. Anyway we quickly came to agree how he would alter my nose and I got clarification from him about anything I did not understand and he was very clear to me on what he can and cannot do to my nose. This way I had realistic expectations. On the day of the surgery he came to see me before surgery and ensured me I would be okay and all will go well, we discussed alar wedge resection before, and he advised that if that is what I want he would do it but he does not recommend I have it done as I do not need it, I listened to him and agreed not to have it done. I am glad that I had I listened to him. He built my bridge using ear cartilage and facia from my temple. Anyway surgery went successful and result were good until after 3 months or more my tip started to drop and columella and tip dropped it went back to its old position which I was gutted about, and I had quite an apparent deviation of the bridge. Mr Matti saw me and suggested to let the nose heal for 12 months and then anything that needs addressing we will look into that. So after a year we agreed to revise and lift the tip again and correct bridge deviation. This was then done and after that my tip did not drop, and bridge was corrected also. Mr Matti also further straightened my septum as originally it was S shaped bent and I was able to breathe perfectly from my nose. I felt Mr Mattis approach is conservative, he tries to not over do the nose so it has a natural look and if further work is required he would then look at what else needs doing. I guess it’s best to be safe than sorry and it is very difficult to predict how the nose will heal. Overall I have had a good experience, I felt I could really trust him, his staff are very professional, he is a very bubbly and jolly character. I felt safe in his hands (after my bad experience with an NHS surgeon it was difficult to have that trust) I have become so much more confidant since my revision rhinoplasty more than I have ever been and it feel great.
