FMLA
I was wondering if anyone can give me advise on how to get my FMLA papers filled out and what can I put on it. I switch to a new pcp and I'm not sure if I want to tell him especially because it's out of the country. Please I need help

Forgot to add on my message .. I didn't tell my doctor out of the country bc I'm sick of the judgement lol . I just said look I'm getting this & this done I have a doctor picked out of state and I would like to know what my options are as far as time off from work bc they want me to use fmla. That's exactly what I said :)

Well this is it. I've been on this site ever since January and now I think it's time I start to document my journey. I'm 35yrs old and a mother of 3 had my first child at the age of 21 so haven't really enjoy that flat stomach and perky boobs I had. Now, it's my time to start living again. I've already bought most of my surgery supply. Paid my deposit and booked a recovery house. Working on losing 10 pounds and building up my hemo. My sister maybe coming the next day after a arrive but not sure. I'm looking for a surgery budding that will be leaving out of NYC.
