I’d always had a weak chin and jawline so decided to do something about it. I researched online the various surgical procedures and options, as well as sourcing the best surgeon. I decided on a custom jawline implant, plus custom cheek implants; and Dr Barry Eppley. I could not help but be impressed with the quality and calibre of his work in general, and especially when it comes to custom facial implants. So, I reached out to Dr Eppley’s office via email to get the ball rolling.
From the initial contact to date I have been nothing but continuously impressed by the exemplary level of customer service provided by Dr Eppley and his fabulous support team. An absolute pleasure to deal with.Since I live in the UK my initial consultation with Dr Eppley was carried out via Skype. He came across as both highly professional and a very nice person. He took time to discuss everything with me in detail and answered any and all of my questions, and he wasn’t impatient and hurried as doctors can so often be. I got the vibe that he genuinely wanted to help me to achieve the very best results. So being very comfortable with everything I did not hesitate to proceed.Prior to carrying out the custom implants design I had to first undergo a 3D CT Scan of my face including existing jaw. This I had done in London, England. Then we carried out the design process. I told Dr Eppley what I wanted to achieve and a first draft design was produced. I found the whole design process fascinating, as computer generated visuals were provided. After 2 additional rounds of tweaks the final designs had been completed according to my instructions and wishes. The it was circa 6 weeks turnaround time to manufacture the custom implants, so I scheduled my surgery date, and booked my hotel and flight accordingly.The time sped past quickly and before I knew it I was flying to Indianapolis. I had booked myself a hotel very locally, wherein there’s plenty to choose from. My pre-surgery consultation appointment with Dr Eppley was on 2nd July 2018 and my surgery on 5th July. It was great to meet Dr Eppley and the team in person, and his workplace is absolutely fantastic. The whole place is like a luxury spa. Very comfortable environment and it certainly helped me to relax and dispel any pre-surgery anxieties. Dr Eppley was as nice in person as he was via video call and took great care to ensure that I was absolutely happy with everything. I had opted for the overnight recovery at Dr Eppley’s hospital clinic.So my surgery was early morning on 5th July 2018. I remember entering the operating theater and saying hello to everyone, and within moments of the anaesthetic being applied I was out like a light. I remember coming around a little disorientated at first. I was in the recovery clinic. I actually felt totally comfortable but of course I had been given pain medication. The nurses were excellent. I was attended to like a king and at no point was I ever anything other than comfortable.Then Dr Eppley came to see me himself and made sure that I was OK and happy. He explained that everything had gone well and that I would like be thrilled at the result of the implants, the cheek implants especially so. I had been warned to expect a lot of swelling, so there was no surprise that there was. However, I was provided with a fully comprehensive array of medications, including antibiotics, anti-inflammatories, pain medication, and an antibacterial mouth wash.The incisions were all small and neat. I had one under my chin which was a lot smaller than I thought it would be, plus two tiny incisions either side of my (new) jaw, plus some incisions inside my mouth, with two in the bottom of mouth for the jawline implant and two up in the top for the cheek implants. The sutures were all self-dissolving so I did not need to have them removed.So, I spent the next few days at my hotel. Everything was fine. I was still pretty swollen but at no point did I suffer any severe pain. In fact, after the first 48 hours I found that I did not need anything more than acetaminophen; and the a couple of days after that I didn’t need any pain medication at all.On the 3rd day after my surgery I was given the all clear to fly home, where I had arranged to take the rest of two weeks off work in order to be able to relax and recover at home.At around 10 days post-surgery about 50% of the swelling had gone down, about 75% after 3 weeks, and by the 7th week almost all the swelling had dissipated.So here I am now, 10 months post-surgery and I am very happy with he results. I did not have any complications whatsoever and Dr Eppley has done absolutely wonderful work. Check out my array of photos, which speaks for itself. I am especially thrilled at the cheek implants, exactly as Dr Eppley said I would be. Overall I look (and feel) at least 10 years younger and I have been getting a lot more attention from girls, which is never a bad thing.That said, now that everything is fully healed I have not got the chin quite right. To be fair to Dr Eppley he did exactly as per my guidelines, so this is totally my bad. It’s a bit tricky predicting how the soft tissue is going to lay over the implant and in short, my chin has ended up too wide. Consequently, I have opted to have the custom jawline implant replaced with a new one of an amended design, wherein I am confident that this will nail my desired aesthetic target. Also, I have decided I’d like to have a chin cleft. So I have already booked my second surgery with Dr Eppley to carry this out. Where as it happens this second surgery is scheduled to occur almost precisely one year from the date of the first surgery. And with all that I have experiences I have to say that not only do I this time have absolutely zero anxiety about having the further surgery with Dr Eppley but I am actually very much looking forward to it. I will be sure to post an update on here following the second surgery and document the subsequent results of the procedures.Either way, if anyone is considering facial implants I cannot recommend Dr Eppley highly enough. The quality and results of his work are both outstanding and amazing. I could not be happier.

Thanks! I think you can achieve better and more natural results with the custom implants because they are customized for you specifically. They are more expensive but are totally worth it!

MrNCA I just want to say that your results are incredible. I'll admit, in the first photos you looked a bit freaky but I knew it was all swelling. But the real post-op photos are amazing. Do you mind if I DM you?

Here's some additional images of my custom cheek implants, as well as my first custom jawline implant, which will shortly be replaced with a second of an amended design.
The first two images show how the custom implants look as overlaid over the top of my own bone structure. I had a 3D CT Scan done which is how my bone structure was mapped for this design and visualization process. This also shows the respective dimensions.

My first surgery, comprising both jawline and cheek implants cost $23,225. My second surgery, comprising only jawline implant (and excluding cost of chin cleft) is $13,720.

This includes the costs of the implants, the surgeon's fees, the cost of the microscrews, as well as the costs of the operating room, anesthesia, aftercare, and recovery care.

I don't know exactly how much it would be for just cheek implants.

Do you feel any pain during smiling, chewing etc? Can you sleep on your side?

And yes I can and in fact do sleep on my side; however, you must not do so shortly after the surgery, so only after you are fully healed

Because of the implants thickness-I guess-you look like a completely different person and not just someone who had surgery.

Was there anyway to achieve your same results but without such a drastic change?

I think you look very handsome but your face looks bigger. I would like to keep my face the same size just with the defined chin, jaw, and cheeks.

What is your oppinion on showing Dr. Eppley a picture of what you are looking for? I know that surgery doesn't mean they can make you look like someone else, I just meant those features.

I have seen two wrap-around implants from Dr. Eppyley and the results seem very similar so I am wonder if all the wrap-arounds will come out looking similar or if it's just the style you all have chosen?

This is such an informative post. Thank you so much for sharing your experience and results. The results look great on you, once the swelling came down.

Do you think that the infraorbital part of the cheek implant made a clear difference or was the improvement in that area more subtle? What if the design had left the infraorbital part out, would the results really be noticeably worse? It seems as if it did not completely eliminate the tear troughs or the under-eye dark areas, but maybe that was not the purpose? Did you have the option to place the cheek implant through the lower eye area?

How did you decide to widen the jaw implant to match the width of the zygomatic arch? Did Dr. Eppley suggest that you should go this wide? Is this common to go this wide? Where you not afraid that it would be too much? I can't decide if this is something I should go for myself.

If I may chime in , I do not think his zygomatic implant design is intended to go right under eyes. I hope he corrects me if I’m wrong. Regarding how wide to go , one mode would be to measure your bizygomatic width and contrast it to average (14 cms to 14.5) I imagine most people would want to be in that range. The issue is hypoplasia rarely occurs in isolation and one may need to make the whole face wider , mandibles , temples etc if they wanted major width gain.

Yes, the infraorbital part of the cheek implant made a clear difference. Specifically, it filled out the area below my eyes which with age had started to become sunken, and thereby has reduced my age appearance. And so, yes if I had missed this part out it would have looked a lot worse, but missing this part out is not an option because this assists the surgeon with respect to the positioning and fixing. Also, you cannot have implants covering over the lower portion of your eye socket so this is positioned across the very top of the bone at the base of the eye socket. Filling out the under eye areas is best done using filler injections, which I might have done at some point.

Regarding the width of the jaw angles, this is something that I decided upon myself. This is something which I considered would for me personally achieve the best aesthetic result. I affixed plumb lines to the sides of my zygomatic arches as part of my design process and did a mockup using photoshop, before proceeding with this particular design feature. So it was not guesswork or a 'stab in the dark', and as such I knew it would not be 'too much'.

Thanks for the reply. Let me ask you this. Since the infraorbital part seems to saddle the orbital rim, did it provide a meaningful vertical augmentation or mostly an anterior effect?
