Uneven Reduction Size, Not Covered?!?

After baby #3, my A cup gradually bloomed to a 34E. It was uncomfortable and starting to cause pain in my shoulders and back. I decided to get a reduction and lift, hoping to keep as much as possible-at least a C, but hoping a D (might as well lift them up while reducing).
After the surgery I was thrilled with the result. After a couple months when all the swelling went down, the difference in size was quite apparent. Almost 2 cup size difference. I wasn’t expecting perfect symmetry, but the difference was significant. When I asked about evening it out, I was told on top of the fee for anesthesiologist, and hospital fee, I’d be charged for lipo 360 to even them out, as I didn’t want more of a reduction on the one, and wasn’t interested in inserting a foreign ‘boob’.
After 9 months, one breast was just under a ‘B’ cup, the other between a C/D cup. Am I justified in thinking this should be a ‘covered’ correction? One would think that a request to have them somewhat symmetrical was a given.
