A Perfectionist's Experience with an Equally Meticulous Surgeon

I first came to see Dr. Karthik Ram around November/December 2025, when I was looking for a plastic surgeon for a breast lift and fat grafting.
Before meeting him, I had already travelled to Bangkok and consulted several surgeons. I had done extensive research, but somehow I never felt completely comfortable enough to proceed. I had previously undergone a double cardiac bypass, so finding a surgeon who was confident and comfortable managing a patient with my medical history was extremely important to me.
I first came across Dr. Karthik Ram through his videos on Instagram and decided to speak with him. His clinic arranged a call, and I remember feeling comfortable with him from that very first conversation.
By then, I had researched quite extensively and had a lot of specific questions. From the way he answered them, I could tell that he was very comfortable dealing with my medical history and knew how he wanted to approach my case. I didn't sense the uncertainty I had felt during some of my previous consultations. That gave me the confidence to come to Chennai and meet him.
I also started doing some facial aesthetic treatments with him. I had previously had fillers and other treatments with different dermatologists and surgeons, but I found that Dr. Karthik Ram had a real artistic touch. The best way I can describe the way he approaches my face is almost like someone sketching a portrait and gradually refining it. He looks at the face as a whole, makes small adjustments and keeps refining until everything looks more balanced. It is never simply about treating one particular line or area.
I knew I wasn't the only one seeing the difference. When I returned to Singapore, I actually had dermatologists asking whether I had done something to my face because I looked very good. They couldn't necessarily identify what had changed; they simply noticed that I looked better. To me, that was one of the best compliments I could have received about his work.
From there, what began as a consultation for my breast lift eventually became an entire body transformation journey. I had my breast lift first, followed by my butt lift and then my tummy tuck.
Going through three major surgeries within roughly a year was certainly not easy, physically or emotionally. I was also travelling from Singapore and was accustomed to a very different medical environment. However, I felt genuinely cared for at his clinic. His nurses and doctors were very caring, and over time I became extremely comfortable with the team.
One thing I came to appreciate about Dr. Karthik Ram is that he is not a surgeon who simply says yes to everything a patient wants. This was particularly relevant with me because I had lost a significant amount of weight following gastric sleeve surgery, had quite a lot of loose skin, and I am admittedly a perfectionist. Once I started addressing one area, I would inevitably notice something else that I wanted to improve.
He was often the person bringing me back down to earth.
He would ask why I wanted a particular procedure, whether I really needed it and whether my expectations were realistic. He was also very conscious that I should have confidence in myself regardless of surgery. At times, I would probably have preferred him to simply agree with me, but this eventually became one of the reasons I trusted him. I knew that if he didn't think something was appropriate for me, he would say so.
My breast lift was the first major surgery I had with him, and my cardiac history naturally made safety a major consideration. I had consulted other surgeons who were prepared to perform a lift and implant together, which was initially what I wanted. Dr. Karthik Ram was very firm about how he believed my surgery should be staged and explained his reasoning clearly.
He was equally transparent about limitations and expectations. He told me what could realistically be achieved and what couldn't. I came to value that honesty because I would much rather have a surgeon tell me something I may not want to hear than agree to something simply because it is what I am asking for.
I later had my butt lift with him, and this was where I really began to appreciate the amount of attention he gives to overall shape and proportion. He didn't simply address the specific areas I had asked about. He looked at my body as a whole and made adjustments based on what he felt would give me the best overall result. There were things I hadn't specifically asked him to address, but he noticed them himself and refined them because he felt they would improve my shape.
I had the same experience with my tummy tuck. He didn't just focus on my abdomen. He improved the loose skin on my thighs and paid attention to my hip-to-waist transition because these areas affected the overall contour of my body.
This level of detail was particularly important to me. I am someone who notices small things, and he took each and every one of my concerns seriously. Even when something was relatively minor, I never felt that it was being dismissed simply because the main surgery had been successful. He would assess it properly, explain what he thought and consider what could realistically be improved.
As a perfectionist, I appreciated this enormously. I could see that he wasn't satisfied with simply achieving a technically good surgical result. He cared about the finer details of the aesthetic outcome as well.
After having three major surgeries with the same surgeon, I think you get a very good understanding of how that person works. Dr. Karthik Ram is firm, and he will tell you when he thinks you are asking for too much. But once he agrees to do something, he is extremely particular about getting it right.
For me, finding the right surgeon was never only about technical ability. With my cardiac history, I needed someone who made me feel medically safe. After major weight loss, I also needed someone with a strong aesthetic eye who could look at my body as a whole rather than treating my breasts, abdomen, thighs or butt as completely separate areas.
That is ultimately why I continued choosing him for each subsequent surgery.
