32DD before with Grade 3 Ptosis. Anchor lift with implants.

After breast feeding and weight loss, I’m left with grade 3 ptosis (the most severe) with significant asymmetry. I originally wanted only a lift, but now I’m adding implants but don’t want to go large.

Sizing thoughts? Also want to add inner and outer thigh liposuction.
I’m having a hard time with the sizing piece because I’ve been large before and I don’t want to be large anymore. I want smaller, perky breasts and it’s hard to use the sizers on top of deflated breasts that I shove into a 32DD bra. He did mention after my lift that I would be a B after removing all of the extra skin and removing some tissue from my left to correct my asymmetry so I’m trying to figure out what a B (if that’s truly where I’ll be?) to a big C or a small D would be CC wise? Anyone else in the same boat as me? Or I should just trust him and his expertise to put in the right size and just give him my “wish photos.” Thoughts?
Also, I’m looking to add inner and outer thigh liposuction and I’ve shared some photos of my saddlebags here. I hear that the lipo recovery is more painful but wanted someone with experience to weigh in. I hope he lets me add it!

Hi there, to share my thoughts on your concerns and questions. I know a number of people here will tell you to “trust your surgeon”. But imo whether to “trust your surgeon” or not depends on who he is, imo. Would you care to share his name? Keep in mind that BL/BA surgery is one of the most complicated surgeries out there not to mention each breast surgery onto itself. I probably spent more time doing research on the different surgeons I consulted with… than I did on the actual procedure itself.
That aside “size” is one of the most difficult decisions to make prior to surgery. Most surgeons usually give you a size range and you basically have to decide whether you want to “err on the small” or “err on the large”. One thing to keep in mind with this particular surgery is the size/weight of the implant will put stress on the breast lift incisions and could compromise the healing process. For this reason my own PS (who is one of the best in the country) prefers to go smaller on the implant size with this particular surgery.
In addition to researching implant type/size you may also want do a little research on breast shape/ratio. I often see is a lot of focus on “bra cup size” (which is not really an accurate measure of the breasts) and not enough attention to breast shape (45:55, 50:50, etc) or areola size (i.e. 38mm). And imo a beautiful shape will render nicer results than a poor shape with extra cc. If you don’t know much about these subjects google “45:55 ideal breast scientific study “and “38mm ideal areola size” to find interesting articles and studies. Not gonna lie - this darn app isn’t the most user friendly (and I work with engineers who design apps!) and it won’t let me “select” his name once I type it in! I’m seeing Dr Richard Tholen at Minneapolis Plastic Surgery! He’s the best of the best with 30+ years of experience and I’m sure you’ve seen him post on here :)
I love your comment about size and impact on the incisions. I haven’t thought about this! And thank you for sharing the ideal study. I have seen people mention it but haven’t read it yet but will tonight! And I’ll jump on over to your profile and take a peek too!! Happy Tuesday!!

I had my breast augmentation with Dr. Tholen on Nov. 8 and he ended up going bigger than I had requested, but they are perfect and exactly what I wanted. I say trust him with the sizing he has the experience and he will listen to your expectations.

“Before” photo update
Updating with some “before” photos I’ve taken on my own to show my asymmetry and my size. I’m still torn on the size but I’m thinking anywhere from 250-300ccs. I’ve been larger before and didn’t love it so I’d be more than happy to be “smaller” (like a D) and perkier. Thoughts on what size I should get??

I know that every body is different, but you look to be pretty petite, like myself. I went from a 30C to a 30DDD with my lift and 300cc implants. I wouldn’t want them ANY larger (they’re about the size I was during the first few months of nursing my babies). If your fear is being too large, vs too small, stress that to your PS (we agreed on 240-300cc implants prior to surgery...and I think I would have been happy with any of them)...my PS, on the other hand, has told me that he wishes I’d let him go bigger

You’re right with stressing I’d rather be too small to too large. When I had my consult he immediately said he thinks 300CCs but I’m so nervous about being too large. When I did he sizers the 284 looked great (even though who knows what it would look like starting from a smaller size) and I even liked the 240 one I tried too. At this point I just don’t want to be a deflated B that is lifted and want something but not big. I definitely trust him and will stress the smaller size. Thanks for the response! I went with 360cc implants. I was a 34d before but they were saggy and had pretty significant asymmetry. I basically let the doctor pic my implants out. I did try on sizers and I knew I didn’t want to be “too big”. I’m still healing right now, but I think they will turn out perfect for my body frame. This healing process sucks tho. I originally had lipo to my abdomen and flanks added to my surgery as well but I dropped those off last minute. I’m SO glad i did. I don’t think I would have been able to do anything at all had I went thru with the lipo. I was expecting a more easy recovery but I suppose everyone is different. You will definitely need help for the first week post op. I couldn’t even open my pill bottles the first two days lol

I have a friend who had a mommy makeover last May and she said the lipo was the worst pain. She said if she would have known it would have been that painful, she wouldn’t have done it. But I also heard that abdomen is much more painful recovery wise than the thighs so who knows??
