Excellent Surgeon, Severely Disorganised Patient Liason

I flew from Germany to Marbella, Spain to have breast augmentation with Dr Kai Kaye. This was to treat a severe size/shape asymmetry and increase size overall in both breasts. I received 420/500cc, over muscle, Motiva implants. 36 AA(left)/B(right) to 36D in both (I know cup sizes are vague. But there was at least a 100cc difference between them). I also received autologous fat transfer from my waist to fill out the breast shape, to reduce the necessary implant size, and to round out the implant edges.
The equalising of the two breasts was accomplished partly by reducing the lower pole distance on the larger breast via removal of some tissue, and partly by lowering the location of the inframammary fold on the smaller breast so that it matched the larger one. Two different implant sizes were then used, with the smaller breast receiving a much larger implant.
RESULTS
I am now 6 weeks post-op and the results are, so far, absolutely fantastic. The fat transfer really makes the implants round and natural looking and the size/shape asymmetry has largely disappeared!
The previously smaller breast is still dropping and stretching as it received a much larger implant in a much smaller space. It is therefore harder to tell what the eventual result will be. It is still somewhat square and situated higher than the right breast. But I am optimistic, and I know that even in previously symmetrical breasts that they change at different rates. I will give it six months.
The right breast looks absolutely fantastic. The lower pole is completely round and has dropped over the inframmary incision. The upper pole is a perfectly flat 45 degree slope, and I can tell that it will curve slightly as more dropping occurs. Completely natural and great looking!
The space between breasts was reduced from around 4cm to 1cm. As the welling has reduced this has become about 2cm.
Overall they look amazing and equal in bras or t-shirt. Hopefully they will match up completely over the next few months.
CLINIC
The clinic in Marbella is small but extremely clean and professional. Everyone speaks English and Spanish. The doctors that I saw also spoke native German. Dr Kaye himself is very friendly and explains what he is going to do in full detail. In fact his remote Skype interview is what convinced me to go with him, as he discussed options and I felt that I was fully listened to. When revealing to him that I am a stealth long-time transitioned trans woman he was professional and took that into account.
The clinic is however lacking when it comes to communication and providing information.
The reception staff / patient co-ordinators are wonderfully friendly, but extremely disorganised. I received no information about what pre-operative cautions needed to be observed until 18 hours before surgery, when I did my per-operative consultation. It was only because I had researched this previously that I knew to avoid certain drugs for the weeks leading to the surgery. Otherwise my operation and health could have been severely compromised.
Dr Kaye himself did not try sizers with me in his office. This may be because I had tried DIY rice sizers myself and came in stating what I wanted quite clearly. But I would have felt reassured if he or a nurse had actually double-checked this with me. It is hard to know if he agreed with me, or felt rushed and didn't want to take time to check sizes.
The nurses are friendly (shoutout to the nurse from the North of England) but also not great with divulging information. I had to chase people to find out what was going on and what I had to do. I received no verbal overview of what was going to happen or what to expect.
After surgery I recovered by booking a night in their accommodation, which came with a night nurse. The room itself was very austere, with no window, just a light well. But it was provided with a en-suite toilet and walk-in shower. There was a non-functioning hospital bed, locked in one position. The nurse call buttons DID NOT WORK. I was given a phone number to call instead (one of eventually 5 different ones that visit). Food was eventually provided from an external restaurant, but not until 9 in the evening, after 36 hours of fasting. The information problem came up again when I found out that there was a kitchen full of snacks and drinks near to my room that I not been told about.
AFTERCARE
I received lymphatic and breast massage from a massage assistant several days after surgery. However I received no information about how to do massage myself in the weeks after surgery. I had to chase doctors and staff for this information, which came down to a one line email saying "squeeze them once a week" which is not good enough.
My largest criticism is that I was not given enough information or equipment to care for the liposuction that took place. I understand now that compression garments are a standard aftercare aid in liposuction recovery. No such garment was provided or recommended by Dr Kaye. I purchased one after I flew back home and it helped enormously.
The pain from liposuction was INTENSE and severely hindered my mobility. On my first night I was unable to sit up without assistance due to pain and swelling. If I had been informed about this I would have planned for events differently (e.g. by asking my partner to stay with me in the accommodation).
I have not yet sent my 4 or 6 week photos to Dr Kaye's office, which I need to do. But I have not heard anything from them inquiring about my status or health, or reminding me to send photos and a progress report to him. For all they know I dropped dead after leaving Spain.
OVERVIEW
Dr Kaye is an excellent surgeon, producing excellent results. But the clinic is unorganised and chaotic when it comes to patient liaison, and the surgeon himself does feel like he is winging it at times. If you are well informed and know what to do, this clinic and surgeon is an excellent choice. If you are unsure, nervous, or dependent on others this clinic would be less ideal.
