Allergan Natrelle Inspira SRF 345 Subfascial Placement
The way my guy friend's described it, "if we could pay 5k to look as good as you guys do with boob jobs, every guy would do it".
I'm 30 years old, never been pregnant or breastfed, don't want children, and work a very active job in medicine wearing a lead suit for hours.
Measurements
Height: 5'7"
Weight: 120lbs (morning no clothes, no food)
At nipples: 33in
Right under breast tissue: 30in
Smallest part of waist: 27in
Biggest part of hips: 37in
Shoe size: 6.5 US (I felt like that was important, good indicator of body build)
BWD: 12.5cm (measured at home didn't ask surgeon)
The initial consultation
I went to a consultation about a month and a half ago with the idea of what I wanted through just looking at photos (400-500cc under the muscle). The surgeon had a different idea in mind. He explained to me that if I pick an implant that's too wide for my frame it will hang out the side and look bad. He also told me about breast implant animation deformity and suggested that I go with the subfascial route because of my decision to wear bikinis and super low tops. I told him I wasn't very knowledged about that technique and would do more research. He said if I wanted to have the implant under the muscle he would do it for me, but he believes my best outcome will be subfascial. He also told me the maximum size he would be comfortable putting on my frame is 345cc in the full profile size. I tried on the sizers from 300-345 and of course I picked the 345 so I can tell myself later when I have boob greed that this is the biggest that was offered. The office staff gave me a list of surgery dates, a quote of the price with anaesthesia, and told me when I was ready to call back and put a deposit down for surgery.
The re-research
I went home and did extensive research about subfascial placement online, spoke with a plastic surgeon colleague from Mexico, spoke with several doctors at work, and also spoke with friends that had their breasts done. Although it is kind of an unnerving feeling to be the only one of my friends to go over the muscle, I truly did not like how their implants did move in person and once I saw it, I could not unsee it. Being a person that would be bothered by that, I decided that above the muscle would have to be the placement.
Sidenote: My family does not have a history of breast cancer. If your family does I would advise strongly against over the muscle implants as the mammographer cannot pull your breast tissue over the implant for compression. In patients with under the muscle implants the technologist will try and squeeze the implant and mucle backwards (posteriorly), your real breast forward (anteriorly), and then compress to see your tissue only.
The preoperative consultation:
This appointment was booked two weeks before the surgery date. They did measurements for the post op bra that they suggested, I played with the implants again and asked 500 more questions, an EKG was done and blood pressure taken, filled out a ton of paperwork, took some pre-op photos. The implant that was ordered is an SRF 345cc Allergan Natrelle Inspira implant. It has a smooth outer shell, the lowest level of cohesiveness, and a full profile. I asked him about the different levels of cohesion and he told me that he likes to use the other two levels for breast reconstruction patients, but feels like they are too hard and people can tell when they give hugs that they are fake. I got my scripts and instructions to stop nsaids now until after surgery and stop drinking a week before surgery. I'll spend the two weekends I have before surgery cleaning up around the house and have purchased button down tops and wedge pillow for recovery. It's very surreal that I'm finally going to have boobs!

Are you happy you chose the responsive? Any rippling? Are they soft?

I don't see any ripples as of now. They're softer than when they were first put in, but not as soft as my real boobs yet. The doctor said it would take at least three months for me to see full soft results though since I had so little breast tissue to begin with and the skin has to accommodate. The responsive is supposed to be good for a more realistic breast. I think you'd be able to tell they're fake if you hug someone with the highly cohesive one because it will be harder.

Thanks very much for your reply! I have less tissue than you to begin with due to old saline implants, so praying I won’t ripple and will have some shape!

Yes I am very happy with the full profile. I don't actually care if my boobs look fake, I just want them to look super nice. Are you going over or under the muscle?

My doctor suggested over the muscle because of my level of fitness and activity. He didn't want me to experience animation deformity.

Day after surgery
Surgery was yesterday at 7 am. I woke up a little after 9 am in the recovery room and didn't remember anything about the surgery. Felt so nauseous that the nurse gave me a zofran, I suppose my body doesn't like anaesthesia.
I went home and stayed in bed all day. The skin is extremely tight and very sore in certain places. Felt some air trapped under my skin as well, feels bad. That should go away in a week hopefully. The night was ok until the nerve block wore off at around 4 am. Woke up and tried to suck it up and not wake up my husband, I woke him up around 6 am and asked him to open the pill bottle so I could take a toradol.
Went for my post op appointment today and the doctor said everything looks ok and to keep wearing the surgical bra or any bra with no underwire. Told me to come back in two weeks. The doctors assistant is very sweet. She helped me readjust the surgical bra so I wasn't being squished as much.
Not moving the pain is not bad at all, when I move it does feel bad, but I've only had that one toradol pill at 6 am and then 2 Tylenol today so far (it's 2pm). Hopefully the pain subsides more. It's rather cumbersome.

Second day after surgery
Woke up this morning in less pain than yesterday which was a relief. Also finally had a BM (sorry about the TMI). I guess my body is finally adjusting to everything. I had taken a toradol yesterday before bed but I haven't had to take any today, just took two regular strength Tylenol around 630 am.
Walked around the backyard today for 26 minutes to get some movement and fresh air, also did some laundry because I felt like I needed to wash the surgical bra. Currently waiting for the surgical bra to dry in the dryer. I find it to be quite irritating that the surgical bra is hand wash and line dry. That's exactly what postop people want to be doing! I'm wearing a cloth bra from Walmart that was greatly reviewed but it still doesn't provide great support which is why it's in the dryer on air fluff.
Hoping that the skin continues to stretch and boobs get a little bigger. The sizers were bigger than my current state but I guess that's to be expected. Hoping I end up at least a C cup :(

Also forgot to add that I was able to shower independently and wash my hair which goes all the way down my back. Yay 20 feet of hair.
