Botox treatment with Dr. Tom Bozek MA FRCP

I did underarms Botox at Surgical Aesthetics with Dr. Tom Bozek on 16 April 2015 while on a visit to London. I paid 700 pounds for 100 units. I did Botox last year and the results were clear from week one. However, with this procedure the results were none. I went to the clinic with full trust. I went thorough the procedure smoothly and without doubts.
On retrospect, the 1- Botox preparations were done away from me. Dr Bozek told me that he used 3 injections. I did see that he used one injection on my right side. I did not see the left side because it was slightly painful.
2- He made me sign a consent form after completing the treatment. I signed at the bottom of the paper. There was a sense of relief on his face once I signed.
3- my reason of writing is that I instinctively feel something is not right.
I asked him if I will be sweat free all summer and there was no answer.
I had his back turned towards me once the session is completed.
He did not give info about the units used. The nurse told me that 100 unit is the standard procedure at the clinic.
After 5 days (20 April), still no results, I had to call the clinic to inquire about his full name due to uncomfortable feelings I had about the integrity of his treatment, the manager (Mrs. Heulwen Nguyen) of the clinic was reluctant but had to spell his name to me. I did Google his name and I found a case of Kate Silverton on the Sunday mail. Her treatment is different but his attitude towards her was the same as mine (no clarity in information, he turns his back to patients once the treatment is completed as a sign of rejection).
I called the manager of the clinic on 22 April –again- and I expressed my dissatisfaction while mentioning Kate’s case, and she accused me for being unprofessional and impatient. She offered me a free top up on my next session. I also requested a copy of my records and the consent form.
I went for a top up on the 8 May at 2 pm. I went in the secretary in the office to let her know of my appointment. The secretary had to call him by phone and she was very formal with me and was not smiling. Heylwen was hiding behind the PC and did not speak to me. There was a feeling of avoidance. I had to wait in the waiting room. A lady came to me while waiting and offered me a calming cream to use before the treatment to show me that she cares, a gesture which did not happen on my first session. Dr Bozek came racing into the clinic and I was called in. He was standing tall to greet me, which I found strange, he made himself trip over the chair as a sign of uneasiness. He was restless while talking, contrary to the first session. The talk was filled with success stores of his Botox treatment to 7 people looking for jobs, I was wondering if these stores were real. He requested me to lie down on the bed for the top up of 2 injections on each side. He also had to change the needle while in session because it went blunt. I think he should have prepared himself before hand. After completing the session I had his back turned towards me. I went down stairs towards the door and none of the ladies spoke to me.
I went to the office and reminded them of my docs. I was given 2 consent forms of the 2 sessions. By the way, I was not given a consent form to sign on my top up session. I think Dr Bozak had prepared the paper with signatures.
The forms are factitious especially the second one, the signature on the form is NOT mine. As of the first form, my signature was in the middle however on the original paper I had to sign at the bottom.
I went again to Google about Dr Bozak and found a guy (real self site) who had unsuccessful facial fraxel. The patient explained fully the treatment in a review report. Dr Bozak conduct towards this male patient was the same as my experience, e.g. changing malfunctioning tools while in session.
I could easily say that the second session is a complete “set up” and the ladies in office were doing their best to cover up for him. His way of conduct is questionable. There is an integrity issue here.
I have the copies of the forms with a clear signature differences, the first form is with a scanned signature and the other is not mine.
On the 18 May I would have complete 10 days to see and feel results of my top up Botox. Unfortunately, there is no sign of great results.

Wow, sorry you're going through this. What are your next steps in getting this resolved? Do you plan to see a different doctor for Botox to help with the perspiration?
