Botox Caused Puffy Under Eye Areas and Severe Allergic Reactions

I have had one Botox procedure with Skin Raleigh before in 2017 with a different injector, I didn't have any issues with it, so I think maybe it is time to schedule another one after a year and half to address some of the fine lines around my forehead and corner of my eyes.
I had my Botox procedure on 4/8, at first I didn't notice any thing wrong. After a few days, I started to notice that my eyes look different, and there was some puffiness under my eyes. Also there are small patches of red and dry skin that showed up on my face.
We had a trip to Atlanta from 4/15-4/19 for Spring break, we took some pictures during the trip but I noticed that my face looked very puffy in the area under my eyes and also there are red patches on my skin.
After we returned to Cary on 4/20 I went to Bond park to watch my daughter perform for the Children's day celebration, on 4/21 my face started to turn bright red and inflamed around eyes and forehead.
I went to visit my family doctor on 4/22 and after my recount of recent activities she was suspicious of two things poison ivy and Botox, she prescribed Methylprednisolone pack for me to take. But my situation didn't improve, so I went to visit my family doctor again on 4/25 and after she saw me she said she is concerned and wanted me to go directly to ER to have it treated.
So I drove right to Duke hospital ER department, the doctor there said that it is most likely a delayed allergic reaction to the Botox treatment and prescribed Prednisone and Hydrocortisone ointment cream. She also warns me against any future Botox treatment and said I should not risk it.
I have contacted my injector twice around 4/25 to seek advice and opinion, but didn't get much help in terms of insight and suggestion to next steps. I was told by her that there is absolutely no chance that my skin reaction is caused by the Botox procedure and I should go see a doctor. When the ER doctor asked about ingredients that was used in the procedure, she gave me the lot number of the botox and soap name that was used.
It took about 2 weeks for the readness to slowly go away. But overall I still feel that my under eye area is puffy and shows up in pictures. I look forward to 2 months later when the chemicals get out of my system. Hopefully no permanent damages are done.
So in retrospect, I believe there are a few things that Skin Raleigh could have done better:
1. Perform a skin allergy test before the procedure, this may help to give early warning as who shouldn't have the procedure done
2. Be aware about the possible side effects of Botox procedures and educate the customers about them, and offering helpful advice and be empathetic with the customers.
For those who are considering a Botox procedure:
1. Be aware of the possible side effects such as puffiness under the eye (if injected around eye area) and severe allergic reaction
2. Ask for skin test before hand (or choose the provider who performs skin test)
I know that personally the negatives outweigh the positives (both financially and physically) so I won't be having another Botox procedure.
