I had the rhinoplasty because I felt my dorsal hump was too large for my face and was unhappy with my profile.
For "good results" I simply followed Dr. Kotler's instructions about cutting out daily vitamins, NSAIDS/Aspirin or any medications & foods that increase bleeding time/bruising (he provides you a long list). Also, Dr. Kotler provides his patients with Arnica to take 5 days prior to surgery as well as Mupirocin to use before. Additionally, he encourages patients to eat pineapple and papaya to decrease bruising pre-operatively and post-operatively. Icing is encouraged in addition to nasal rinsing frequently.
Being informed with the entire process is best, which Dr. Kotler does a great job at explaining. He also provides his patients with the Kotler nasal airway after the operation so that you can continue breathing through your nose -- making the process even more painless. Packing and the airway was pulled the day after surgery, which was painless and I was able to breathe through my nose with ease afterwards.
Regarding pain, I was given Tylenol #3 which I took every 8 hours the two days after the surgery. After that I took tylenol for any discomfort (which I only needed a few times). I had absolutely no noticeable bruising, just some swelling at the dorsum and the tip (which improves every day). The cast was removed day 5 post-op. The only thing I was restricted from was heavy exercise, for 10 days because Dr. Kotler states that it increases your blood pressure and can increase bruising. But I was able to take long walks, enjoy my Christmas holiday with my family and it did not stop me from participating in any activities within 1 day after surgery. I had my surgery on the 23rd, had my packing pulled the morning of the 24th, showed up for Christmas dinner in the afternoon, drove to Flagstaff, AZ with my husband the night of the 24th, spent a few days in Flagstaff. While there we took long walks in the woods, visited the Grand Canyon -- the surgery did not stop me from anything!
I will add the "new nose" photos in a few weeks.

My experience with Dr. Kotler & his staff could...
My experience with Dr. Kotler & his staff could not have been any better. I have been fully satisfied and tremendously impressed from my initial Skype consultation through post-op. A California native, who is temporarily living out of the state, I was able to schedule my Rhinoplasty when home over the holiday without any problem. The staff was more than flexible despite the season. My initial connection with Dr. Kotler’s office was with his patient consultant, Dolla, who was very hospitable and informative throughout the entire process. Dolla assisted me in setting up all necessary consultations (I utilized the Skype consultation and also had an office consultation before the surgery) as well as my surgery date. Dolla was always available to answer any questions and continued to be diligent at keeping me educated about timing and the process throughout.I had done my research on several facial plastic surgeons, and had both phone and office consultations with several – but none of them made me feel as comfortable as Dr. Kotler. After my initial Skype conversation with Dr. Kotler I was impressed with his character and knowledge base and the office visit only made me feel more confidant in my feelings about him. After meeting Dr. Kotler face-to-face, I knew that I did not want anyone else operating on my face. And he was the last office consultation on my list of other Physicians I was considering. Before my surgery, Mary, Dr. Kotler’s nurse, took care of all of my Pre-Operative needs. Like the rest of the staff, she was always available, more than willing to answer my questions via email and was continually responsive. Mary was even willing to come in on the day of Christmas Eve, when nobody else was in the office, to pull my packing so my husband and I could get to our family event. Such an above and beyond favor. The surgical center was beautifully neat & clean, the staff was amazing and the whole process was quiet unbelievably easy. Being my first surgery I was unsure what to expect – but could not have been more pleased! The whole surgical staff was marvelous as well. The anesthesiologist called me pre-operatively to discuss my procedure and make sure I was not nervous, as well as someone from the surgical site as well.
I am two weeks out post operatively; I did not have any bruising, just some minor swelling. My nose looks fabulous (so far – it is still healing) and I would tell EVERYONE I know who is considering a similar procedure to do it at this office. I have never been so impressed with a physician, his staff and facilities.
