I had 18 year old textured saline implants under the muscle. I was diagnosed with scleroderma about 3 years ago. My implants were old and I no longer liked the way they looked, plus I think there is a chance that my scleroderma may improve when they are out. Either way I wanted them OUT! I also wanted good results. I knew I needed a doctor with experience in removing implants. I did a lot of research and scheduled 3 different consultations. Two "top" plastic surgeons in the bay area told me that I would NOT like the outcome. They both said that I had very little breast tissue, would probably be an A cup at best, and insisted I would not be happy with the results. The third Dr. had great reviews, but was out of state and I was very nervous about his age - 78. Then I started seeing some very scary reviews (with pictures). So my search for the right doctor continued.Along came Dr. Cassileth, a leading breast specialist. Dr. Cassileth is known for her skill in fixing other surgeons' mistakes as well as breast reconstruction work. According to Dr. Cassileth, breast implant removal is becoming a growing trend among women. She sees many patients who want to go back to their natural breasts. You can "google" her and find videos of her discussing implant removal. She has a procedure where she can "rearrange tissue" and she also does fat transfer if needed. I scheduled a consultation that was over a month out, but the wait was worth it. During my consultation, she spent near an hour with me. Before she actually looked at my breasts, I told her what I had been told about having "very little tissue" to work with. When she examined my breasts, she said that I had plenty of tissue and would be about a B to small C cup. I was so happy!  She explained how she would perform my procedure. (Everyone is different and she acknowledges this). I scheduled my explant and lift. It was expensive but well worth it. I was confident that she had the skill and felt like I would be well taken care of by Dr. Cassileth and her entire "team". She is very personable and her bedside manner is a very caring one. My confidence in her was well placed. She is the BEST! She is an artist and truly an amazing person. I am so happy with my outcome! I think I look better now than I did before the implants. I am now, about a B cup and perky! But the best thing is that they are real and they are mine. It feels so good! I cannot thank her enough. Dr. Cassileth's "team" - all the way from receptionist, medical assistants, surgery coordinator, OR nurse, anesthesiologist, to Post Op nurse were top notch. I also had a "dent" in one breast from a previous lumpectomy - she fixed this too! I am so pleased! I highly recommend Dr. Cassileth. She rocks!!

I am so encouraged by your post because I want my implants out, but have been told the same negative comments about how they will look after removal. When you say expensive, what would I be looking at (surgery, length of stay in CA, etc)? I honestly think I would like flat as a pancake better than fake and ugly and uncomfortable; however, if I could find a surgeon who actually specializes in making women look better after implants, I'm in! My experience with implants has been nothing less than a nightmare. I wish I had realized there was a website like this before I ever got implants. Why don't more plastic surgeons offer their clients some alternative natural routes to better looking breasts? I never heard about fat grafting or internal lifts until it was too late!! I hope I don't end up looking like the bride of Frankenstein once this is all over with!! If I didn't have a sense of humor, I don't know where I'd be (mentally and physically). I should have paid attention years ago in abnormal psychology when I read that women who get breast implants are more likely to commit suicide!! Seriously!

swjunkie101 - I was scared of what I would look like too.....until I found the right Dr., I knew as soon as I found her that she was the right one. You should google Lisa Cassileth. She is very skilled in what she does and she believes that everyone should have breasts they can feel good about :) As far as the price goes...first let me say that she is worth every penny spent. Removal starts at $12, 000. If you need fat grafting or lift that is additional. I had Removal with total capsulectomy and a lift for $18,000. Are you ill from your implants or do you have capsular contracture? She will submit to insurance. She doesn't guarantee they will cover all costs. I'm hoping my insurance will at least cover the removal. By the way mine were textured saline under the muscle - can be tricky to remove because the textured implants like to stick to the ribs. She did a fabulous job! I traveled from Northern Ca, a 6 hour drive. Stayed in a hotel the night before surgery. Surgery was scheduled for 7:30 AM. Thursday. She has her own surgery center and I was out by noon and back at the hotel with drains. The next morning went to her office, had drains removed and was cleared to go home. I think she likes to keep her patients close though. So, if you are coming from out of state - she'll probably want you stay a few days. I wish you the best, I know exactly what you are going through. Just know that it is possible to have your own breasts and love them too :)

Recovery from explant and lift has been a lot easier than recovering from breast implant. I have not even had to take any pain killers :) I am sore but I had textured implants submuscular removed that were stuck to my ribs. I required repair to my pectoral muscles at the time of explant. Doctor said no heavy lifting , pushing or pulling for 6 weeks due to muscle repair. I am at day 13 and still somewhat sore but swelling is down and incisions are healing up nicely. The hardest part is not doing anything too strenuous.

Wow...sounds like a great experience with a great result. Just curious about the lift you had...did you have scarring or did she do an"internal lift". If it was an internal lift, did she use ADM or any kind of mesh to help support your breasts? I had my implants taken out 9 months ago on and they could really benefit from a lift....just worried qbout scarring. Thanks so much for your story.

Hi! I know it`s quite an old post, but I`m considering Dr. Cassileth for my explant surgery and there are not many reviews on RS about her. Please, it would mean the world to me if you can PM me some photos of you before and after explant! Did you have en block/total capsulectomy? How long the incision is? I read for en block it`s like 9 cm long incision which really freaks me out. I think implants make me ill, I`m prepared for smaller breast (or more like no breast) but huge scars scare me. I have below aureora scar from BA but I don `t think they can do en block this way. Also, did you have internal lift? Is it efficient?

Hi there! Thanks for your review. I’m from the Bay Area as well and am highly considering Dr. Cassileth as well. Can you tell me what hotel you stayed at? I know she recommends people to stay in a care facility the day following the procedure but that is probably way expensive. How was your drive home? Did you have drains? Thanks

The aftercare facility, Serenity, is AMAZINg and actually a bargain for what you get. Attentive care, incredible food— honestly, I was freaked by the cost, too, but realized in one hour it is a BARGAIN!!!
