Dr. Paz left me toothless with trouble eating,...

Dr. Paz left me toothless with trouble eating, smiling and talking normally. The custom made device with a pontic that I had was removed and now does not fit anymore after 10 days of lingual braces services from Dr. Paz. He took impressions for my braces without removing the device, he did not get ortho clearance by a dentist before ordering the braces and didn't realize I was not a good candidate for lingual braces because not only I was still fighting periodontal disease but I also had a maryland bridge for years and the back of my teeth was scratched off already deeply to fit the bridge. Now the teeth are sensitive to the touch, to the air I breathe and I have been and I am in a lot of pain. Dr. Paz left me with a ton of glue on the back of my teeth and general anesthesia is required to remove it now. I also developed periodontal disease under Dr. Paz's care, which was discovered after the impressions were taken. I had to undergo an emergency extraction, bone graft and deep cleaning and had to spend thousands of dollars. After all of that, he forced me to bond braces 6 months after impressions were taken, without retake new impressions after an extraction and after bone graft, otherwise he said he will charge a few extra thousands of dollars to have the case redone. A few days after he bonded braces and glued and unglued on one side a pontic without any brackets support, I had to call 911 because my jaw started moving involuntarily and and the pontic crumbled in my throat in my sleep and I started chocking and cpr was performed. Braces were taken off but plenty of glue remained. Doctor left me toothless and in a lot of pain. Tooth needs to be replaced, more periodontal disease developed due to the glue attracting bacteria and I am left with many more bills to pay. I also gave Dr. Paz almost $3000 that he kept in his pocket. He never sent me to an implant specialist and when I confronted him about it he said there was no guarantee an implant would fit after orthodontic treatment while he didn't even bother make sure I was a candidate for an implant while I was told I needed braces to fit an implant by him. Turned out I do not need braces and I am not a good candidate for an implant nor lingual braces nor braces in general with active periodontal disease. Currently Dr. Paz refuses to provide me with the paperwork regarding the lingual braces he ordered for me and the pontic. He is denying me access to important details, specifically the dates and details and paperwork sent to the laboratory to create the braces and the pontic.
