Sorry - I just realised you said these were taken after surgery. Do you have any pics from before surgery? What were the issues you had fixed? I can still see some alar notching - but perhaps this is better than it was before?

All images were taken after the last revision Dr. Guyuron did. My case was very difficult. My nose was upturned and the shape of the tip was poor. Dr. Guyuron used grafts to elongate the nose, which I believe is one of the most difficult rhinoplasty surgeries to perform. I don't believe I had alar notching before but that might be a not unexpected side effect of making the nose longer, I could imagine. No perfect nose can be expected with a difficult basis to work from with previous scarring, imo. But I feel he performed a miracle. To say I highly recommend him would be an understatement. I can't think of any other surgeon that would be able to do what Dr. Guyuron can do with noses. (Although one surgeon once told me he could, and I had to concentrate to give a decent and polite reply to that and not grin a bit). His staff is awesome too.

What can be done to fix the misshapen nostrils? I have the same thing after rhinoplasty. Are these before or after your last rhinoplasty?

Dr. Guyuron wrote about alar retraction in some papers from years back advising amongst others to use alar rim grafts and composite grafts. Not sure if he still uses them in surgery though: he never mentioned it to me. Perhaps since he does not think there is retraction.

Will fat injections be the definitive answer????? If so, I am shocked. I've had fat injections in the past on my face, not only did it do nothing, the results are always temporary..... I did look like I was run over by a truck for about six weeks after. That's all I got from fat injection. Folks , do your homework

Don't get why the surgeon would suggest fat injections either. That is never going to help with such notching. To me this looks like a nose that could use revision, not like a nose after revision. The nostrils look biggish and the shape not good. If this is the standard for revision with a famous doctor I am worried for revision.

No way in a million years would fat injections help. It is a liquid injection that does not resemble cartilage. The doctor was blowing her off.

The price suggests you at least got two surgeries? Several patients I have read about that had surgery with Dr. Guyron seem to have more revisions. If you had, do you have pictures after the 1st revision with him? You got a nice improvement. But I agree with what TJC24 said about the alar notching and your tip seems still somewhat upwards. Has your columella thinned out? Not everyone can afford multiple surgeries so it would be nice to know if you had one or more revisions with Guyuron.

My nose is collapsing after his revision. It is pinched and my nostrils are slits. He overressected the life out of nose. It looks like a 70's nose job and it is turning into the something so unnaturally small it is scary.

Sorry to hear. Did he use any grafts? You might want to look into Dr. Toriumi. Some patients seem very averse to his practice but I think a lot of his work is nothing short of fantastic. His approach is to build up noses, not to remove too much cartilage as patients complain about quite often after surgery it seems. The nose might look a bit bigger for some time after surgery but there is a lot of contraction/shrinkage for the first years and his surgery anticipates that. Certainly when grafts are used and when he performs revision surgery. A too sculpted nose is not that enviable and feminine [RS bleep] imo; a nose that looks not operated on at all and with a good structure, sufficient de-rotation of the tip and with some softness can really complement the face and look graceful. His price is high but he usually makes a drastic change and gives a very good result in one surgery so in the end it might not make that much of a difference. He has a lot of experience with tip de-rotation and alar rim grafts too if that is what you need. Best of luck.

Please do yourself a favor and go talk to Dr. Rick Davis in Florida. My daughter had an overressected nose. She had almost nothing holding her nose up. This was about two years ago that she had revision by dr. Davis. I worry all the time that something is going to move, shift etc. but so far so good. He used rib cartilage. I am forever grateful to him.

My daughter lives in Chicago. She saw Dr. Toriumi and with all his negative reviews, we were to scared to go to him. It would have been nice in the same city but not worth it for us.

To all of you out there in need of revision. PLEASE DO YOUR HOMEWORK, THERE IS ABOUT FOUR DRS. IN THE COUNTRY THAT ARE KINOWN TO BE THE BEST IN REVISION. IM NOT EXACTLY SURE OF ALL OF THEM. HAVE DONE ALOT OF RESEARCHING.

Could you email me her before and afters. It would be very helpful and also if you could pm me. I am very interested in getting a parent's perspective because both my parents are against my revision right now and don't see my issues.

Both open rhinoplasties which seems like the most rational approach for revision surgery to the tip imo. The surgeon would want to be able to see the structures/scarring.

I was asked if I had a recent picture and if the nose still changes. It does still change I feel even years after the last revision if I compare pictures. I think it will continue to change as is the nature of rhinoplasty revision (in general I think there is expected to be some ongoing scar contraction, perhaps some partial resorption of grafts and grafts get more visible since some swelling can linger for years). This picture was taken just days ago.
