Initial Consultation
I decided to stay on one of the quieter streets in the Khao San Road area. This may seem crazy, as Khao San’s known for partying and nightclubs, but I figured it would be a really easy and convenient place to get food and supplies, with 7-11’s and pharmacy’s open 24 hours and tons of restaurants serving Asian & Western food including lots of vegetarian options, which can otherwise be tricky in Thailand. My boyfriend and I stayed in a nice hotel with a rooftop pool for 1000 THB /night and had the consultation with Dr Montien booked for the day after our arrival.
The 200,000 THB fee for the surgery included all our transfers, so we were picked up from the airport by someone from the World Medical Center and taken to our hotel and picked up the next day to go to the initial consult.
We were greeted at the entrance and taken upstairs. The hospital is huge, white and clean. There were not many patients around, but plenty of friendly staff who spoke English. We went into a small consultation room with Dr Montien. First we talked about my nose, what I don’t like about it. I said it was too big, a bit too pointy, a little bulbous at the tip, the line of my bridge is not smooth and I have a slight hump. I showed him pictures of noses I liked, which was a really helpful resource. I’d been saving such photos into an album for months! He looked at my nose from every angle and then he dropped the bomb…
He said my skin is relatively thick, particularly on the bottom third of my nose – the tip. This means that swelling will be prolonged and could require steroid injections for several months to aid it because thick skin does not suck in against the new nose shape very well. Swelling will be worst if he attempts to bring the tip closer to my face, making my nose smaller. He says it could be very swollen for up to 6 months. He also says that in some rare cases people receiving the treatment he’d need to do on me can be swollen for up to a year, he said it’s very rare, but that the few people that are like this look like me, have my complexion. I felt like I’d crashed into a brick wall and I had a big decision to make. He offered an alternative; he could smooth out the bridge and remove the hump, where the skin is not as thick and just leave the tip as it is. I tried to keep calm and think carefully about what to do. After some thought, I realised if I just get the bridge adjusted and not the tip, I think I’m gonna be back here in a couple of years asking for a revision to do the tip, as the tip was the main thing I wanted to change. He said it would be far better to do it in one operation. I asked him to paint me a picture of my worst case scenario in terms of the swelling/healing timeline and with some serious consideration, I decided I was willing to take the risk. It felt really full-on and scary, but I’ve wanted a nose job since I was about 12, I’ve finally saved the money, done all the research, come all this way, I wasn’t about to back out now. I said “Let’s make a nose.”
He took photos of me from different angles and we looked at them on the computer and discussed the changes. He said he could remove the hump, lower the bridge to make an elegant curve from the arch of the eyebrows down into the bridge of the nose and bring the tip closer to my face. He was very honest and upfront with me – he said because of the thick skin, he couldn’t give me really petit nose, but it would be smaller, the tip closer to my face and the bridge more elegant. He said if bringing the tip closer to my face made the nose look too wide, he would perform alarplasty, which would leave a fine scar hidden in the crease where the nostril meets the cheek, but he would make that decision during the operation. After an hour and 45 minutes with the doctor, the consultation was over and I was happy with the plan. Two days until surgery!

I spent several months researching rhinoplasty in Bangkok. After narrowing it down to my top 4 picks of surgeon, I decided I’d like to go with Dr Montien. I thought about booking it through a cosmetic tourism company, but in the end decided that I would try organising it myself, as it would be more affordable and I heard lot of Westerners do it, so I figured the hospital would be used to dealing with patients direct. I’m really glad I did it this way, because it was actually quite easy!
I submitted my assessment photos through a web portal called TopDocs. Dr Montien replied with great specificity on the issues he saw with my nose and what procedures he’d do. He also said it looked like the skin on my nose was medium-thick, which would mean he could give me a smaller nose but it would prevent him from being able to give me a very petit nose. I appreciated his frankness and so I requested to book an appointment through the portal. I got no answer, so a week later I requested an appointment through the World Medical Centre website. I got a reply the next day from the friendly International Correspondent at WMC, who apologised for the lack of response through the portal. Only the initial request was ignored - after that the portal was really useful and staff responded quickly through it.
I let them know which dates I’d be available and they offered me a date for the initial consultation with the surgery the day after. They said usually it’s best to book 3 months in advance, but they had a cancellation, so were able to see me in one month’s time.
I wanted to get Belkyra (Kybella) for my chin too. It’s not available in Thailand yet, nor in New Zealand where I’m from, so I decided to go to Thailand via Australia, where it is available. I confirmed the appointments and booked my flights! Six days in Melbourne visiting some friends and family and getting my chin injected, followed by 16 days in Bangkok.
I wanted someone to come with me to help look after me, so I asked my boyfriend. He was not too keen on me getting surgery in the first place, but he could see I was really serious about it and agreed to come with me and be my nurse! <3
I got super budget flights for us – Jetstar, Scoot and AirAsia, so that was quite cheap. I decided to book a hotel in the Khao San Road area – not for the nightlife, obviously! But because I figured it would make things very easy in terms of getting food and supplies. I found a hotel that was pretty affordable, but had a pool on the roof and was in a quiet street near Khaosan, called Rambuttri Village Inn. The hotel was good – the room was really nice and the hotel was clean and being in the backpacker area was very convenient.
I continued doing lots of research about what to bring with you and making albums of noses I liked and disliked to show Dr Montien!
