The issue is that the applicators currently available do not fit the arms well so treating this area usually is not possible. CoolSculpting treatments require the use of one of three available hand pieces. The desired treatment area must have the proper contour and the fat must be pliable enough so the fat can be pulled up inside the applicator. Rumor has I that the company that manufactures the CoolSculpting device is developing additional hand pieces, which would allow treatment of additional areas such as the arms and thighs. It as of yet they are not available.

Coolsculpting on the arms is an off label indication since it is approved for the flanks. Also, the applicators typically do not fit well on the arms. Proper patient selection is critical and we do not perform Coolsculpting on the arms in our practice.
Zeltiq - effective for slimming arms in the right patient
Remember that Zeltiq gets rid of fat, but it won't do much to tighten the skin. Therefore, Zeltiq is very effective at sculpting the arms if:
- there is pinchable fat that will fit into the zeltiq template/probe AND
- the arm skin has good elasticity
Patients with lax arm skin should consider these options:
- Brachioplasty (surgical removal of fat and skin from the arm) or
- laser liposuction (the heat from the last results in some tightening)

At this time, treating the arms would be off-label and physicians have found it difficult to fit the current applicators to the arm area (there is talk that Zeltiq is making a new applicator for the arm area). The best way to know if to have an examination and see if you have excess tissue (not just skin) that would go into the applicator to get a good result. Physical examination is the best way to go.
CoolSculpting by Zeltiq can treat the upper arms in some patients as an off-label use
Although the company has not yet distributed applicators that are designed for the upper arms specifically, the small applicators currently used for the love handles and stomach can be used as an off-label indication for other parts of the body if the physician determines that it is appropriate. The arms however would have to have an excess of fat rather than just loose skin for it to work.
