Converting gastric sleeve to bypass - Have a substantial umbilical hernia: Is a combined surgery possible? (Photos)

In 2013, at approx 600lbs I had a Gastric Sleeve. I've lost over 200lbs since, however, I feel the sleeve has reached the end of it's capabilities. I now feel I need the second stage procedure, the bypass, to finish the weight loss.I chose the sleeve because I was aware that the severity of my obesity could lead to more complications, or worse, if I were to have the longer and more complicated, bypass. However, I was also always aware that it may need to be a two-step process, which I now believe is required.I have a substantial umbilical hernia, which I had prior to the sleeve and is stable, but somewhat restrictive. Is it possible to have both surgeries carried out at once? Would it require open vs laparoscopic? Anything else I should consider in making this decision?I have attached some photos of my hernia from the outside and from the inside, taken during my sleeve surgery.
