Melasma can not be completely cured, however effective treatment are possible. Dermal melasma is one of the hardest form of melasma to treat. This is because your skin is extremely sensitive to UV and even the smallest amount can stimulate your pigment cells to produce colour, additional the pigment lies deep in the skin- ie. Dermal. The mainstay of treatment is strict UV protection- hats, sunglasses and SPF every 4 hours.
I prefer to use a combination of creams and laser for melasma. Hydroquinone can be used (5-8%), along with vitamin A creams. I usually start my patients on laser (low dose Q switch or Picosure) a few weeks after they commence on creams. Picosure Focus is my laser of choice, followed by the Revlite or C6, then the Clear and Brilliant. For dermal melasma, its Picosure as my first line.

I have all of the lasers imaginable to treat melasma but I would encourage you to seek out a physician who treats the skin with Cosmelan for melasma. It is extremely effective for melasma, especially for patients who are worse from past laser treatments. Best, Dr. Green
