Hi, Docs. I have atrophic box-type scars in my temples. Those were caused by severe acne years back. What is the best tool to use? Subcision? Infini? Fillers? Can I also ask if you know the model of Infini machine pictured here? That is the machine that will be used by my specialist. Does it have a different settings? Thanks!

Answer: Infini for acne scarring Infini is a great treatment for acne scarring like yours. I would highly recommend it. The machine in the picture is not familiar to me. I had to send it around, and I was told it is an old model with the old logo. I have no idea whether it is substantially different from the current model or whether the settings would be any different.

Answer: INFINI can work, but subcision spacer filler is a better option . Short answer is that I have seen INFINI work -not a primary method, but the needles essentially break down the scar tissue vertically and by secondary intent your scars are freed up. In your case I would probably perform subcision with a spacer filler as you have not only atrophic scars but tethered scarring as well. Leave it up to your specialist for the settings as he or she will know best. Infini is not the best treatment, it is only one of many treatments that we can employ to treat your acne scars. Too many patients rely on the device as THE treatment, it is not- its only a tool that the Specialist uses to sort out a problem. Acne scar revision is a sub-specialised field. For the best results, one should target the acne scar type with ideal treatments and not just one device. For example deep ice pick scars, and narrow box car scars can be treated with TCA CROSS peels, mixed scars, rolling, and atrophic scars treated with fractional devices such as Fraxel, fractional lasers, PRP and INFINI radiofrequency.
