Pending CT Scan for further discussion on treatment plan. GFR is 11, Fistula surgery is done, KIV for dialysis as patient's condition is okay. What are the risk factor for surgery in view of patient's health conditions and age factor? Recovery process?

Wide excision

You kidney issues aside, the photo looks like you need a wide excision and skin graft to heal that neglected SCC. It should not have been allowed to grow that large before excision.
