Narrow upper with protruding front teeth.

You problem stems from more than just thumb sucking.  Your jaws are massively underdeveloped.  

AVOID ANY DENTIST WHO WANTS TO EXTRACT FOUR TEETH!  

Look into a DNA Appliance and Epigenetic Orthodontics/ Orthopedics and with Oral and Myofunctional Therapy.  You have probably been using your tongue improperly your entire life.  The tongue creates wide arches.  You might also want to be evaluated for an ankylotic tongue whick is often a reason  for this type of development.  Do you snore or have sleep apnea?

You can grow a full wide healthy physiologic smile and occlusion without surgery or extractions.
