I am a 50 year old female, 5'3" 110 lbs. Never smoked, no alcohol or drugs. I recently had trauma to tooth #18 while biting into something unexpectedly tough. This tooth has a 12 year old root canal and has had 2 different crowns over that course. I have had on and off pain since the injury and 3D Xray shows some inflamation possibly at the root. Though it could be scar tissue. I can eat on the tooth fine, but deep pain when flossing between it and 19 which is an implant from a failed root canal 20 years ago. I consulted with the top oral surgeon in my area aboit possible extraction with implant to follow. He also performed 3D Xray there and pointed out that it would be quite risky as there is maximum of 2mm margin for where the implant would be placed and where the nerve is located. To further complicate things, I have a very small mouth with restricted opening. He said given the limited amount of space if it were attempted it would most likely need to be done by hand as the space does not allow for traditional needed tools if I understood him correctly. He said he I most likely would be fine without replacing the tooth. I really hate the idea of not having all of my teeth. I don't have the xray, but have included a fully opened mouth photo. If the extraction proves to be necessary, I will be having a bone graft and PRF, but is the implant too risky and difficult to persue?
