Question

3 years ago I received Botox in my forehead, and immediately had a bump above my eyebrow (on my right side of my face). The bump seems to have a border around it, and was originally soft but seems to have become more firm. Dermatologist said it was too deep for him to assist with (at the muscle). GP had an ultrasound done and said nothing showed, so he said it was scar tissue. What options do I have?

Benign, non-structural thickening (sometimes perceived as scar tissue even when it’s not a true scar in the classic sense)
Less commonly, a small lesion that is below the resolution of ultrasound or blends into surrounding tissue

Because Botox acts on muscle activity and not skin structure, it wouldn’t directly create a permanent deposit. So when something persists this long, we usually think less about the product itself and more about how the local tissue responded afterward.

A firm bump that has been present for 3 years is unlikely to be active Botox at this point, since Botox itself wears off within months. The timing may make it feel related, but a persistent firm area usually needs to be evaluated as its own issue.

Since you already had an ultrasound and nothing concerning was seen, the next best step would be an in-person exam with a board-certified dermatologist or facial plastic surgeon who can feel the area and decide whether it behaves more like scar tissue, a small cyst, localized fibrosis, or another benign soft tissue change.

Treatment depends on what it actually is. Options may include observation, a small steroid injection if it is scar-like/inflamed, laser treatment if there is redness or surface texture change, or minor surgical removal if there is a defined lump. I would avoid trying to massage or dissolve it without a clear diagnosis, especially this long after the original treatment.
