Reverse Tummy Tuck blood supply - is there an increased risk for necrosis?

I am a major weight loss patient. Overall, ~175 lbs. In 2020, I had an FDL tummy tuck but due to the amount of skin and some minor extra/unplanned weight loss, I have a good bit of excess skin above my belly button. It tends to cause physical pain in many circumstances. My original surgeon, when I asked at a follow up in 2020, said they'd do a reverse TT. I am at a point in which I am looking at getting the skin removed (along with my bra line/back skin) and have been getting consults. Another doc also says reverse TT. The outlier (so far, I have another couple scheduled) is one that told me he would only do it as a revision, following my vertical incision from the FDL. The reason was due to already altered blood supply and increased risk for skin necrosis. I'm trying to make a final decision on surgeon/procedure, and need to figure out if this is something I should be concerned about, considering only one surgeon has mentioned it. Thank you
