48-72 hours after surgery are usually the worst days in terms of inflammation, pain, and swelling.  With a tooth #4, that area typically is not too involved with the sinus and if it is, it is usually just the tip of the implant in most cases.  Of course, we don't have an xray of your case so I am just giving info on what we generally see.  Did the dentist perform a "closed" osteotome sinus lift with your implant (did the Doc tap on a mallet when putting in your implant)?  If so, this has been known to cause some post-surgical vertigo.  As for the meds you are taking, they are pretty good meds for any surgery involving the sinus.  May add some Afrin into the mix.  I would certainly call your Doc and let him/her know what is going on.  They will likely keep you on those same meds for their course.  They are good meds.

its my advice to upload post op xray , so i can guide u better. there r chances of sinus membrane tear. Do not blow your nose for next 4 weeks. Do not sneeze holding your nose. Sneeze with your mouth open. Avoid dusty environments. Diving in water and flying in aircraft should be avoided. Avoid lifting objects by bending, blowing up balloons or playing musical instruments that increases nasal or oral pressure.
