Why are facial scar removal procedures or surgeries not covered by health insurance?

I understand that rhinoplasty's and plastic surgeries/ and or changes to the face for "improvement" are definitely cosmetic in nature. But "normalizing" the appearance of a scar shouldnt be considered cosmetic. And I am only concerned about facial scars. I have burns and scars on my body that I could care less about that have no real affect on daily life. I have a face scar from a surgery when I was young. So why isnt facial scar removal considered cosmetic if it doesnt improve looks but normalizes then?
