Question

I had a double mastectomy with double incisions and free nipple graft on April 26, 2016. Today is May 15, 2016. I am concerned about the status of my nipples. Are my nipples dying?

The free nipple graft technique is essentially a skin graft technique.  The grafted nipple/areola relies on the blood supply from the skin on which it is laying.  Therefore, it is not uncommon to have partial or incomplete "take" or healing of the grafted tissue.  The scabbed area will fall off, but the skin behind will heal in leaving a scar that sometimes resembles an areola.  There are options.  Be patient. Nothing to do about it at this point.

It is hard to tell from the photo, but if they were put back on as a free nipple graft, they often go through a period of peeling and crusting as they heal. If concerned, then it is best to discuss with your surgeon.

You may be experiencing nipple loss especially on the right.  It would be best for you to go see your surgeon for an examination.
