I mean when a doctor is used to doing tummy tucks/lipo on patients with large amounts of excess skin/fat, would someone who has very little skin/fat be very different or more challenging? Or would this be easier or the same...is it riskier for that patient with less fat/skin?

A liposuction used to treat stubborn areas of fat in someone who is thin, will give a very nice result, accentuating the physique of the patient. A true tummy tuck is a skin loss operation an not really a fat loss operation. You are removing the excess skin and pulling it taught, and at the same time, tightening the abdominal muscles.
