Was consulted for mild A cup tubular breasts, goal large B cup. He suggested internal releases and a lower implant for a more natural shape, which may increase the risk of temporary or permanent nipple sensitivity loss. Online, I found standard implants have a 5-7% risk, while with releases, it could be 15-20%. Are these estimates accurate? Are there other risks with releases? How long could sensitivity take to come back? Can damaged nerves heal, or is it irreversible? What are the statistics?

Changes in sensation are a risk with breast surgery.  Patients may experience some changes during recovery, with sensation slowly returning as healing continues and swelling resolves.  Most regain sensation before one year.  Changes that are present one year post op or longer are usually permanent.

Most women who undergo breast augmentation surgery will have some decreased nipple sensation. The amount of a change in the personalization varies based on a number of different circumstances. Using very large implants seems to increase the loss of sensation. Women do not lose sensation, completely but often experience a change in nipple sensation that is slightly decreased.
Some change in nipple sensation should be expected.
Breast implant, augmentation puts breast, tissues on tension, and this can impact nerve function. Just like your foot goes numb if you cross your legs for too long.
Most patients get used to that change in sensation, and as long as they’re aware that this change is likely to happen or still happy with the outcome, and the change in nipple sensation by itself typically does not cause overall dissatisfaction with the procedure.
Women who were not informed about the potential change in nipple sensation may be disappointed or upset, because they were unaware of this inevitable consequence of having the procedure.
I suggest any patient contemplating having cosmetic surgery have multiple in person consultations before scheduling surgery. All question should be answered, and patients should be comfortable with having a good understanding of most aspects of the procedure before scheduling.
The approach or choice of incisions do not seem to have a significant impact on the change of nipple sensation, but the choice of implant size does.
