Should I really remove 2 perfectly healthy upper first bicuspids for orthodontic purposes? (Photos)

Hi, I'm 38 and my crooked upper teeth are messing with the lower ones and causing pain and wear. The doctor wants me to remove 2 upper first bicuspids to align and then pull everything back to correct for overbite. My misalignment is mostly unilateral . After seeing how much the doctor struggled to get the perfectly healthy tooth out I'm really struggling psychologically to get the other one out since this should be enough space to correct my crooked teeth without causing mid-line asymmetry.
