The Best Treatment from the Best Team!

I always get the best treatment at PUR-FORM! I first learned about Dr. Purita when I was having trouble with my knees. I had the max-stem procedure, and now I can do everything I want to do again!
Everyone at the office is always so kind and helpful, and they are also knowledgeable about all of the treatments.

Results Second to None!!! Gave me my smile back without the volume of fillers

My experience with Emily and PUR-FORM was better than I could have imagined. I'm 57 years old and the extent of my cosmetic treatments was Botox every couple of years. Brand New Facility with Exceptional Services

I've had 3 Morpheus treatments so far and Emily and MJ have been amazing. They both explain things in details that are easy to understand. They review the before and after care required when getting Morpheus treatments. I'm always relaxed and know that my skin is in great "hands" with Emily. I highly recommend Pur-Form when considering any of the treatments they offer.

Fabulous on all accounts, skin is starting to take on a younger, glowing look

Emily performed the PRF facial on me at the end of May, 2023. The facility is spotless and the staff is extremely professional. They took all the precautions prior to the procedure. They prepped my skin properly, and they explained every step along the way. Emily is very knowledgeable in her field. When the treatment was over I was sent off with supplements and creams to reduce any side effects. I was slightly swollen the next day. That is all. As the days pass, I see noticeable

Emily was amazing! She is so knowledgeable and patient. She made me feel comfortable and the experience was enjoyable. I will absolutely be going back to her! Also, she checked in on me too the next day which was unexpected but a pleasant surprise.

Detail oriented perfection! Botox for wrinkle reduction & in masseters for TMJ

I've been going to Emily for Botox in my forehead for wrinkle reduction and in my masseters for TMJ and it has helped me SO much! I recently started doing Morpheus8 and PRP/Biofiller and am obsessed with the results as well. I like to maintain a very natural look and she always listens and executes perfectly. I'm getting married soon and trust her fully for my aesthetics needs leading up to the wedding.
