Thanks for your question. While I can't give specific advice by internet I can make some general comments. It really depends on the patient's goals and expectations. An implant alone will definitely restore some fullness (although 150-175 cc is a very small implant). But it will not lift the nipple too much.
A lift will elevate the nipple but will add to cost and will increase scarring significantly.
If there is some drooping or sagging to the breasts then over the muscle is definitely recommended.

Thanks for the question. The final decision regarding size rests with the patient but there are some limits and, of course, some advice that your plastic surgeon can give you.
I commonly tell patients: "the bigger you go the faker you'll look". Now some patients don't mind a "fake" look. Also it is mainly without clothing that they will look less natural. Clothing can hide a lot!
If someone is an A cup and they are petite then, in my opinion, a 415 cc implant is on the large side. I think a 300-375 range would be advisable. However it is ultimately your own decision and I would take the advice of your own plastic surgeon as well. Hope this helps.

Hello and thank you for your question! This is a very personal decision but I can give you some advice in general terms. First of all, you have chosen a fairly conservative size which is perfect for your dimensions. Secondly, there is not a huge difference between 255 and 285cc. It is literally 2 tablespoons. Thirdly, as a rule of thumb, if deciding between 2 implant sizes that are close together, nearly always best to go with the larger one. More women wish they had gone bigger than smaller.

Thank you for your question. There is no right answer to this - most plastic surgeons (including me) have a particular preference. In my opinion, I have made a custom pocket for the implant and I don't want an external garment to push things where I don't want them. I also do the surgery with cautery and under direct visualization so bleeding and swelling are not big issues.
Therefore I use no bra at all initially, just the dressings and a loose top. I see patients 5 or 6 days postop and at that time I recommend a loose-fitting sports bra of their choice for 3 weeks. At the one month mark they can where whatever they like as long as it is comfortable.
