You have significant herniation of fat and the two out of the three fat pockets in the lower lids. This is also called a prolapse. Do not have extra skin based on these limited photographs. We do not recommend fillers placed in this area due to significant complications that can happen such as blindness. Also important to get your allergies under control with nasal sprays, antihistamines and decongestants. A trans-conjunctival approach lower blepharoplasty can accomplish removal of these fat pads as an outpatient procedure under general anesthesia, which takes about 30 minutes. The incisions located completely on the inside of the lower lid, and there's no visible external incision. That's going be your best solution.

Polly beak can be caused from two issues, thick skin and residual dorsal/tip cartilage. To treat thick skin require steroid shots to reduce the swelling in the supra tip area of the nose. If the poly beak is caused from excess cartilage, then a revision rhinoplasty is required. To prevent the tip of the nose from drooping down dynamically when smiling requires releasing of the depressor septi ligament.

From the two limited photographs, it appears that most of your fat is located above the platysma muscle in the neck, therefore liposuction should be able to accomplish your goals. If you're looking for more jawline definition, a platysma- plasty can also be performed, which significantly improves your jawline once the fat has been removed. If you're fat deposits are located below the muscle than a formal neck lift surgery is required.

A closed rhinoplasty approach can accomplish shaving down the dorsal hump, reduction of your hanging columella, refinement to the nasal tip cartilages, and osteotomy placed in the nasal bones to narrow the bridge line and close the open roof that's created from the hump removal itself. All of the incisions are placed on the inside of the nose, and there's no external incisions needed, and no painful packing required either. An Alar plasty is performed to narrow wide nostrils, not lower them. Functional surgery, such as a Septoplasty and turbinate surgery can also be performed simultaneously on the inside of the nose for improvement of airflow dynamics. Rhinoplasty is the most difficult procedure to perform correctly in the entire field of cosmetic surgery, so choose your surgeon wisely based on extensive experience producing natural results.

As long as there's no infection, and the implant has not been displaced, it's best to leave well enough alone. There's no reason to go in and just to remove a screw. In our practice, we use silicone chin implants manufactured by Implantech, and we secure the implant in place with a dissolvable stitch that takes about two months to go away. We usually perform chin implants under local anesthesia as an outpatient procedure, which takes about 30 minutes. They can always be removed if necessary.

My Experience Was over the Top!

My experience was over and above what I would expect. I was terrified of changing my face and terrified of the surgery and recovery. The entire staff, especially Dr. Portuese made me feel safe and sure about the procedure. I am more than pleased with my results having had neck and chin (face lift) surgery. I would highly recommend if you are considering any kind of plastic surgery to consult with Dr. Portuese and his staff before anyone else.
