Dr. Gary J. Alter has been a double-board-certified plastic surgeon for nearly three decades. One of the first physicians to be certified by both the American Board of Plastic Surgery and the American Board of Urology, Dr. Alter specializes in genital plastic surgery, including genital aesthetics and reconstructive procedures. His bicoastal practice includes locations in both Beverly Hill, California, and New York City. Over nearly three decades, Dr. Alter has performed over 10,000 genital surgeries and pioneered both male and female aesthetic genital procedures, as well as gender confirmation surgeries. He has invented many surgical techniques that have been adopted by other plastic surgeons in the field, including the Alter central wedge labiaplasty (aka the Alter “V” labiaplasty), which preserves the original labia edge and reduces postoperative pain. Dr. Alter received his medical degree from the University of California, Los Angeles School of Medicine, where he also completed a surgical residency. He went on to complete a urology residency at Baylor College of Medicine in Houston, Texas, and a plastic surgery residency at the prestigious Mayo Clinic in Rochester, Minnesota. He was also selected as a fellow for the genitourinary reconstruction and plastic surgery program at Eastern Virginia Medical School in Norfolk, Virginia. Demand for Dr. Alter’s expertise in genital anatomy and cosmetic procedures has led to frequent appearances and interviews with media outlets such as The New York Times, The Los Angeles Times, The Wall Street Journal, “Botched,” “Strange Sex,” USA Today, Allure, Self, Glamour, Marie Claire, Cosmopolitan, “The Larry King Show,” and more. A: As the inventor and innovator of the wedge technique in 1994, I reconstruct many women who have had labia reductions by other surgeons, both wedge and trimming techniques. The reconstruction technique depends on the deformity. The reconstruction is more difficult than a primary labiaplasty and should be done by a plastic surgeon with extensive experience in labia reconstruction. I published the first and most extensive paper on labia reconstruction in the prestigious journal "Plastic and Reconstructive Surgery" over ten years ago. You will need various reconstructive techniques to give you a good appearance Your left labium can be improved with a small wedge excision to make a smooth transition between your frenulum and upper labium. I would also remove the extra fold on the left side of your hood. On the right, you had over-excision. I believe you probably have adequate skin on the side of your clitoral hood to use to reconstruct a good part of your labium to make it match the left side better . I invented and published this technique in that journal article. In this surgery, I take the remaining tissue of your clitoral hood and transfer it downward to create labia minora. I have done this surgery about 100 times with excellent results. The surgery takes about 3+ hours so it is not a simple office procedure. If any surgeon reduces more of your clitoral hood, you will be ineligible for this repair. Most of my labia surgeries not are reconstructions of botched labiaplasties. A: There are many treatments such as laser and now stem cell injections including nonfat. Fat has a lot of healing stem cells. With nanofat, fat is liposuctioned then liquified, which saves the stem cells. The liquid fat with stem cells is the injected into the areas of LS. A: The breast tissue can be removed through an incision around much of your areola. Hopefully, the skin will contract requiring no further surgery as your skin looks tight. If you have adequate clitoral hood, your labia minora can be reconstructed with clitoral hood flaps, which I invented and published in that journal article. In this surgery, I take the remaining tissue of your clitoral hood and transfer it downward to create labia minora. I have done this surgery about 80 times with excellent results. The surgery takes about 3+ hours so it is not a simple office procedure. If any surgeon reduces more of your clitoral hood, you will be ineligible for this repair. A: As the inventor and innovator of the wedge technique in 1994, I reconstruct many women who have had labia reductions by other surgeons, both wedge and trimming techniques​. You had a trimming labiaplasty which can result in asymmetry, excess clitoral hood, scalloping, and tenderness. Persistent pain and tender areas are not uncommon with trimming labiaplasties. I often reconstruct these cases with multiple wedges or replacing the scar with normal-unscarred tissue. I may use multiple wedge excisions to reduce your labia, eliminate many of your scallops, and improve symmetry. Sometimes, I use skin from the sides of your clitoral hood to partially of totally replace the labium. The reconstruction is much more difficult than a primary labiaplasty and should be done by a plastic surgeon with extensive experience in labia reconstruction. I published the first and most extensive paper on labia reconstruction in the prestigious journal "Plastic and Reconstructive Surgery" about ten years ago. You can have a much better appearance by a very skilled reconstructive labiaplasty surgery along with elimination of the pain. In my hands, the pain is almost always eliminated or much better. Approximately 50% of the surgeries I perform are revisions of other surgeons' labiaplasties. You only get one good chance to reconstruct you, so be patient and ask a lot of questions. Gary J.
