Weight loss can significantly affect breast size and may therefore alter the appropriate implant size for you .

At the very least you need to be reviewed by your surgeon .

A sizing session with sizers and imaging on the Vectra 3D would be of great benefit .

The best way to be sized if with a combination of trying sizers and 3D imaging with systems such as the Vectra 3D.

Submuscular placement may initially be a little flattened by the overlying muscle .

But in the long-term the implant behind the muscle may even look a little larger as placing the implant behind the gland can lead to some thinning of the gland . This is more common with larger implant sizes however .

You are petite and both of those sizes are appropriately small to suit your frame . The difference in the two sizes is so minor that it would be difficult to recognize which was which .

So all in all I think 3D imaging is most likely to help you .

It is hard to be definitive in advice just from those photos but it may be that a combination of liposuction to the side and fat grafting to the border of the jawline can improve your contour .

See your surgeon again and discuss what more can be done .

If this is a major problem then switching to in front of the muscle is the only effective solution .

However if you are thin then this may make the implant more visible in the upper breast . Placing the implant in the subfascial position is likely to get you enough coverage but reduce animation .

Sometimes subsequent fat grafting can also be of benefit .

If you find the right surgeon you may be able to do both procedures on the same day .

I commonly combine these procedures for my patients ; it shortens their recovery and gets it all over at once .

Other than that the order of surgery is down to personal preference .
