Dr. William is a great doctor but I wish I had more opportunity to speak with him in person after surgery. I can’t get a hold of anyone to answer my questions and I went to my one month post op and didn’t get to see my own doctor. I am 9 weeks post op and I have indents in my stomach area. Not flat like I hoped I would get. Shape is great but I’d like to know why my body looks like this.

A: You can certainly perform another lift and based on your photos I think it would be appropriate. It is possible as well that you may have a capsular contracture that can be addressed at the same time as will as decreasing the implant size. Most likely you would also benefit from a capsulorrhaphy as well.
A: Symmastia literally translates to “one breast.” It is a condition that can be present from birth but is usually acquired after breast implant surgery. The problem usually comes from being too aggressive trying to get tight cleavage. Mild ones can usually be corrected by downsizing to a smaller implant and suturing the capsule closer to breast bone down to the ribs. More severe one generally require a mesh or acellular dermal matrix (ADM) to repair.
A: Diastasis is a weakening of the abominable muscles that can lead to an unsightly bulge. It is common after pregnancy, large weight fluctuations and aging. EmSculpt is a high frequency electromagnetic that causes a supramaximal (more than you can do on your own) muscle contraction. Four sessions increase muscle mass by 30% and this is proven with MRIs done before and after the treatment. Generally, mild diastasis (such as shown in your picture) can be corrected with EmSculpt leading to a flatter and more firm abdomen. Moderate and severe diastasis will likely need an abdominoplasty (tummy tuck) to correct.
A: Personally I think this way of picking an implant is backwards. I know it is the most common way to pick an implant where the size is decided prior to surgery, but I prefer to make the final decision in the operating room by using a sizer. By using a sizer we can see in real time in the operating room how the shape of the breast looks. To get the best cleavage and the best shape I like to create the pocket and fit the implant to match the pocket like a hand in glove. It maximizes the most pretty aspects of the breasts. I have found you get the best shape to the breast with pretty cleavage and a beautiful look by matching the implant to the breast. I then place the same implant size as the sizer. In this way the number of the implant is not really that important and the focus is put on the shape of the breast and the cleavage. It is impossible to know how the breast will look just by picking a size from a photo, or holding an implant or even in my experience using software. I know it is old fashioned to do it this way but I have found over the years that if the focus is on the shape and not the number patients are happy and their breasts are pretty.
A: It doesn't appear to me you are bottoming out, rather your breasts are falling off of your implant. In other words there is a disconnection between your implants and your breasts. We want the implant to be centered under your nipple but it appears your breasts nipples are lower than the center of the implant and a lift would be needed to return your breasts to the correct position. A lift would reposition your nipple to the correct spot and a lift would also reshape your breast to give you a rounder look.
