This 23 yr. old pole dancer had saline implants under the muscle against my advice because she had widely placed breasts and pectoralis muscle insertions. She chose the advice of others to go under the muscle for a more "natural" look. The muscle forces from pole dancing created bilateral herniations in three months. Suturing the muscles back down and creating a new pocket on top of the muscle with silicone implants, with an imbricating technique of the hernia I devised was recommended. She refused silicone, so the same implants were relocated on top of the muscle, and the new imbrication technique was used. Her result was respectable, but it would have been even better with silicone gel. She returned to pole dancing without a recurrence, proving the sub-pectoral placement caused the original herniations. There are ways of fixing these complex problems without the use of mesh.
