Background: This 42 year old man consulted with me to resolve his tired and puffy eyes. He noted that the bags were causing him to feel self conscious and worried they might worsen with age.

This is a very common concern with patients in their late 30’s through early 50’s. The combination lack of sleep, emotional stress and age, can cause eyes to look puffy, saggy and darker in color. This can occur in two ways: One is an excess in the fatty tissue underneath the eyes, causing the fat to bulge and give the appearance of puffiness and swelling. This can best be resolved with Blepharoplasty surgery, which removes the excess fat and tightens the skin beneath the eyes. The second is a loss of fat from the cheeks and midface which can deepen the tear trough and pre-orbital region, making the bags appear larger. This can be resolved non-surgically using Eye Dermafillers such as Restylane and Radiesse to replace the lost volume. In this case, Blepharoplasty surgery was needed to remove the excess fat, but the surgery did not resolve a small amount of remaining puffiness, as well as the darkness of the skin beneath the eyes. I followed up the surgery with Dermafiller treatment to refine his results, and they added the perfect finishing touch. This is a great example of how surgery and Dermafillers can be used in conjunction to provide optimal results.

Procedure: Lower Lid Blepharoplasty surgery, 1.0cc Radiesse to refine results

1 Month Follow-Up: The skin beneath his eyes is smooth and tight, and there are no more signs of dark circles.
