This 41 year old woman had 4 previous surgeries by several plastic surgeons. She was not happy with her breasts because the the left breast crossed the midline (synmastia) and was completely different than her right breast. Synmastia breast reconstruction patients present difficult issues and this surgery should only be approached by a reconstructive surgeon who has seen this many times because anything less than a formalized plan will leave the patient in either the same condition worse. When the patient looks down, the mismatch is very visual. She also has a right breast that is somewhat high and can use some improvements. In order to take care of the synmastia, the scar around the implant must be removed and contoured. The muscle can also be of help to define this area. The implant must be taken out on the left and the capsule laterally modified such that the nipple becomes more well placed on the breast. She is too full in the midline on the left because the implant is doing all of the power pushing because there is no resistance anymore to the implant coming to the midline. Both breasts modified and she is seen approximately 4-6 weeks post-op.
